UPDATEFDA Drug Safety Communication: Safety Review of possible increased risk of blood clots with birth control pills containing drospirenone  [May 31, 2011]

… The European Medicines Agency (EMA) announced on May 27, 2011, that it is updating the product information on oral contraceptives containing drospirenone and ethinyl estradiol regarding the risk of venous thromboembolism after review of all available data, including the same newly published data FDA is reviewing.

The FDA is currently evaluating the conflicting results from these studies and will look at all currently available information to fully assess the risks and benefits of drospirenone-containing birth control pills. FDA will continue to communicate any new safety information to the public as it becomes available.

UPDATENovember 2009 status "snapshot" of ongoing YAZ safety study called "International Active Surveillance Study of Women Taking Oral Contraceptives (INAS-OC Study)".

YAZ (3 mg drospirenone/20 mcg ethinyl estradiol) is an oral contraceptive (OC) which is the first pill to combine 20 mcg of ethinyl estradiol (EE) with the so-called "fourth generation" progestin drospirenone (DRSP).  YAZ was approved by the FDA in March 2006.

A company press release, "FDA Approves YAZ(R), The First Oral Contraceptive To Offer Drospirenone In A 24-Day, Active-Pill Regimen", issued at the time of FDA approval, can be read so as to suggest that the list of possible serious side effects of YAZ about which women should be most concerned does not include any cardiovascular side effects:

YAZ contains 3 mg of the progestin drospirenone that has antimineralocorticoid activity, including the potential for hyperkalemia in high-risk patients, comparable to a 25-mg dose of spironolactone. YAZ should not be used in patients with conditions that predispose to hyperkalemia (i.e., renal insufficiency, hepatic dysfunction, or adrenal insufficiency). Women receiving daily, long-term treatment for chronic conditions or diseases with medications that may increase serum potassium should have their serum potassium levels checked during the first treatment cycle. Medications that may increase serum potassium include ACE inhibitors, angiotensin-ll receptor antagonists, potassium-sparing diuretics, potassium supplementation medications, aldosterone antagonists and NSAIDs.

In fact, the current YAZ package insert, or label (accessed 5/18/09), includes this same text in bolded font, while the warning about cardiovascular side effects like deep vein thrombosis (DVT), pulmonary embolism (PE), heart attack, and stroke is not bolded for emphasis.

Seemingly, however, the FDA has had some concerns about an association between YAZ and DVT, PE, heart attack, and stroke, as seen below.

A study called the International Active Surveillance Study of Women Taking Oral Contraceptives (INAS-OC), which was started in August 2005 and continues to date, is intended to evaluate the risk of those cardiovascular side effects for women who use DRSP/EE birth control pills like YAZ.

According to a May 11, 2009 article, "ACOG 2009: Preliminary Findings Favor 24-Day Regimen of Drospirenone and Ethinyl Estradiol for Contraception", published online by Medscape:

[P]reliminary findings from the International Active Surveillance Study of Women Taking Oral Contraceptives (INAS-OC) study were presented here at the American College of Obstetricians and Gynecologists (ACOG) 57th Annual Clinical Meeting by Jürgen Dinger, MD, PhD, from the Berlin Center for Epidemiology and Health Research in Germany….

"The INAS study is a transatlantic prospective cohort study that was requested by the [US Food and Drug Administration] to investigate the safety of a 24-day regimen of 3 mg DRSP and 20 μg ethinyl estradiol (24/4 regimen)," Dr. Dinger told the audience. To date, the INAS study has enrolled 52,219 women….

The study's objectives are to compare the cardiovascular safety of the 24/4 regimen of DRSP/EE to established oral contraceptives (OCs) during standard clinical practice (eg, deep venous thrombosis, pulmonary embolism, acute myocardial infarction, stroke); to investigate the incidence of rare serious adverse events associated with the use of 24/4 and established OCs; and to investigate contraceptive failure rates associated with the use of the 24-day and 21-day regimens of DRSP/EE and all other OCs.

For women using YAZ, here is some important information about the symptoms of these serious cardiovascular side effects from the WebMD.com web site:

This medication may rarely cause serious (sometimes fatal) problems from blood clots (e.g., pulmonary embolism, stroke, heart attack). Seek immediate medical attention if you experience: sudden shortness of breath, chest/jaw/left arm pain, confusion, coughing up blood, sudden dizziness/fainting, pain/swelling/warmth in the groin/calf, tingling/weakness/numbness in the arms/legs, headaches that are different from those you may have experienced in the past (e.g., headaches with other symptoms such as vision changes/lack of coordination, existing migraines becoming worse, sudden/very severe headaches), slurred speech, weakness on one side of the body, vision problems/changes.

A quick Internet search for the word "YAZ" will show among the results that there are discussion forums with "threads" like "WARNING ABOUT YAZ BIRTH CONTROL PILLS", at the SteadyHealth.com web site, where numerous women have reported having a DVT or PE while using YAZ pills.

All of this leaves us wondering whether YAZ is a possible unsafe birth control method, like Ortho Evra and NuvaRing….

    My 25-year old daughter was recently admitted to the hospital after going to the ER b/c of intense chest pain, numbness in her arm and shortness of breath. After ruling out a heart attack, the doctors quickly determined she had a PE. A CT scan revealed a large clot in her right lung and three smaller clots in her left lung. This has been a shocking situation. She’s active, has been in otherwise excellent heath and has had no previous health issues. Genetic tests indicate the PEs were not caused by hereditary factors. Doctors suspect YAZ caused her condition. She’s suffered horrible pain; she’s very scared about the long-term effects and/or a potential reoccurrence. She will be on Warfarin for the next six months. Her doctor tells her not to to expect to feel well for at least the next 8 weeks. It’s sad and frustrating.

    I am sorry to hear about the pulmonary embolism (PE) that your daughter developed while using YAZ. It is a good thing that she got to the hospital when she did because, as you probably know now, a PE can be fatal if it goes untreated.
    Of course, we hope your daughter fully recovers as quickly as possible.

    On July 27, 2007 our 23 year old daughter collapsed suddenly and died while taking a walk with her fiance. Her autopsy stated that she died of an undiagnosed pulmonary embolism. She was beautiful person both inside and out. She had just graduated from college and was engaged to be married to the love of her life. She was a Division I swimmer and had excelled both academically and athletically. The coroner suggested our immediate family have extensive blood studies completed to rule out a clotting disorder. All of our results were negative. Our daughter had been taking Yaz for approximately 8 months.
    As we approach the second anniversary of her death, there seems to be increasing reports of PE’s associated with Yaz use. I of course wish I knew then what I know now. Our lives will never be the same. Thank you for listening.

    I hope you, your family, and your daughter’s fiance are carrying on as well as can be expected following the sudden loss of your daughter.
    As an attorney, I want to make sure you are aware that the majority of states have a two-year statute of limitations, or filing deadline, for wrongful death lawsuits — meaning that if you have not already done so, any claim against the drug manufacturer for legal compensation must be filed before the second anniversary of your daughter’s death.

    I am a 34 yr old woman with a history of menstrual and ovarian problems. After 6 months of bleeding, horrible pain & incredible weakness, I underwent a D&C to clear out my uterine lining and an endocsopy to check for endometriosis.
    A small polyp was found in my uterine lining but no endometriosis.
    I was placed on Yaz to promote monthly bleeding and to mitigate PMDD and menstral pain.
    I’m not looking at a calendar, but I believe it was during my second pack of Yaz that I experience the most terrifying “episode” while driving with my daughter.
    I was suddenly stricken with visual disturbances, followed by left sided hand numbness that climbed up my arm, into my face & even included my tongue & teeth. I had my baby with me so I did not go to the ER. The episode lasted about 45min-1hr.
    Two days later my PCP ordered a brain MRI which was read as normal. I have NO history of migraines or “episodes” – at least not until I started Yaz. I have not been able to get to the ER during an episode because my infant daughter is always with me but my doctor believes I am either have mini-strokes or now have migraines.
    Since the initial episode, I have had two more and they are getting progressively worse. The 2nd episode was in July and it lasted several hours. It too began with visual disturbances, followed by left sided numbness & peaking with a headache and fatigue.
    The third episode started with the visual disturbances (more severe this time: I couldn’t even see that my hands were attached to wrists), progressed with left sided numbness & followed by a 4hr migraine. I could not open my eyes nor move.
    Again, I have NEVER suffered from migraines or even regular headaches. I believe I started Yaz in May, first episode was w/in 6wks & they are becoming worse.
    I cannot believe medications such as this one continue to make it to market. I am terrified that I am having mini-strokes or that I am at risk for blood clots, etc.
    I am stopping YAZ immediately.

    Gosh, this is scary. I have been taking YAZ for the past 3 years now and I thougth it was a God-send. I had such terrible emotional problems the last two weeks of my cycle, so much so it was interfereing with every aspect of my life. When I started taking YAZ, I was much more level throughout my cycle. I still have some problems a few days during the last week but it has been great. This worries me…but alas, I am going to start trying to have kids soon so I guess I am lucky?

    I’m so glad that I found this page! I was considering taking YAS again, not sure if it might have been the cause of my prior problems. But now I’m convinced that YAS is not worth it. I used it for 6 months and i had such bad numbness on the right side of my face, arm and hand. At one time I was unable to write an exam! Also after some blood tests it showed that my liver was severly damaged.
    I eat very healthy,exercise and I don’t drink alcohol. Since, I’ve not been using YAS for 9 months and I haven’t experience the numbness and my liver is healthy. So, no to YAS!

    I’m a 24 year old South African female with no prior health problems whatsoever. I’m fit and healthy and today I was diagnosed with DVT. I now start the long process of taking Clexane injections and Warfarin to try and recover. The only change in my lifestyle recently was a switch to Yaz 2 months ago. My doctors suspects it could be the Yaz as I’m not the first patient of his with DVT at my age, and on Yaz. Yaz has only been available on the SA market for 8 months so the support against it is not really in place yet. Has there been any progress in the States regarding lawsuits or further medical trials on YAZ and it’s link to DVT?

    In the U.S., there are about 3000 lawsuits filed against Bayer alleging personal injury or wrongful death.

    I am a pharmacologist and a clinician by trade and although I am aware of the ongoing safety trials for Yaz..I am more concerned regarding the contraceptive failures I have seen with Yaz and Yasmin in the last 5 months. Six documented pregnancies in women who “state” were adherent to taking their OC’s every day at the same time. One of these was a 21 year old, who became pregnant on Yaz with twins and she had a septate uterus. Both fetuses were in one side of her dual uterus. At 14 weeks her uterine wall ruptured causing the contents of the uterus to form an amniotic fluid embolism. Which then led to cardiovascular and respiratory arrest. No..she didn’t make it…but here is one case where Yaz failed and inevitably led to a young womans death. Devastating for her husband and her family.

    I have not (yet) received any such reports from women using Yaz / Yasmin / Ocella / Gianvi / Zarah (DRSP birth control pills).

    My wife was diagnosed with psuedotumerdiease after taking yaz for short period of time and still isn’t right have you had any cases like this one?

    With regard to YAZ and the other drospirenone-containing birth control pills, e.g., Yasmin, Ocella, etc., our legal cases are limited to the following side effects:
    •Blood Clots
    •Deep Vein Thrombosis (DVT)
    •Pulmonary Embolism (PE)
    •Stroke
    •Cerebrovascular Accidents (CVA)
    •Heart Attack
    •Myocardial Infarction
    •Gallbladder Disease / Injury (cholecystitis)
    •Gallbladder Removal (cholecystectomy)
    •Death
    This is not to say there is no association between your wife’s use of YAZ and her diagnosis, and that is a medical issue which she should take up with one of her doctors.

    I am a 38 year old female based in South Africa, who went onto Yaz about 2 years ago as recommended by a dermatologist for acne vulgaris. Had not taken any sort of chronic medication before this. I am a fairly healthy, active person (work out 5 days of a week and have annual full medical assessments). Suffered with shortness of breath for the past 4 weeks which was said to be bronchitis. Collapsed last Thursday after a scary episode that involved failure to breath, cold sweat and dizziness. Tests later confirmed pulmonary embolisms across both lungs, it seems to be many clots, of varying sizes. Have been in hospital on Clexane and Wharfarin since last Thursday and blood is thinning though at a slower rate than the specialist would like. This drug really needs to be taken off the market! In my ward is a 36 year old lady who has just come from ICU, also with pulmonary embolisms who tells me she’s been on it for 8 months. This cannot be coincidence. Too scary but glad to be alive and in treatment.

    I am glad you are currently recovering from your side effect.
    As regards the legal situation here in the US as regards incidents like yours: At the present time Bayer is only negotiating cases where the date of injury or death was before April 2012, given the defense lawyers’ position that a label change the drug company made that month distinguishes the “later” cases from the pre-April 2012 cases as regards their level of legal liability.
