Three NC Hospital Cases Subject Of Article In Annals Of Internal Medicine

On January 20, 2006 the Annals of Internal Medicine published online an "early" release version of an article describing liver problems associated with the antibiotic Ketek (telithromycin).  The print version of this article will be found in the March 21, 2006 edition of the medical journal.

In the three cases described in the Annals of Internal Medicine article, patients at Carolinas Medical Center, in Charlotte, North Carolina, developed differing degrees of liver problems after taking Ketek.  One patient died from liver failure two weeks after taking Ketek for five days, another patient required a liver transplant after using Ketek, and a third person developed drug-induced hepatitis after Ketek therapy but later recovered.  The authors of the article point out, however, that these three cases do not prove that Ketek caused the liver problems.

In connection with the early release of the Annals of Internal Medicine article about the three North Carolina cases involving liver problems after Ketek use, the FDA issued a MedWatch Alert concerning Ketek and possible liver problems.

The antibiotic Ketek is manufactured by Sanofi-Aventis SA, and was approved for the U.S. market by the FDA in April 2004 to treat acute bacterial bronchitis, sinusitis, and pneumonia.  Ketek is part of a group of medications known as macrolides that includes erythromycin, Pfizer Inc.’s Zithromax, and Abbott Laboratories’ Biaxin.  To date, there have been rare reports of liver problems associated with the use of Ketek and these similar antibiotics.

As quoted in a January 20, 2006 Wall Street Journal article, John S. Hanson, one of the article’s authors, tried to put the possible Ketek and liver problem "link" in perspective by stating:

"These cases could represent an unusual clustering of a rare, idiosyncratic drug reaction at one medical center….  However, the severity of liver injury in two of our patients warrants this report in the medical literature and will alert other physicians to this possible link with [Ketek]."

Meanwhile, Reuters reported on January 21, 2006 that Sanofi-Aventis says Ketek’s "safety profile" remains unchanged from their perspective.

    I suffer from chronic lyme disease. I was treated with IV antibiotics for 8 months with some sucess to starting to feel better until I was taken off IV’s and put on oral ketek to “continue” my ttreatment.
    I became very ill with what seemed to be undefined symptoms leading us to believe that it was a relapse of the lyme. I was finally pulled back off antibiotics and the ketek was stopped because of elevated liver enzymes.
    I now have NASH–non alcholic steato hepatitis. this was diagnosed by biospy last year. My liver is severely infitrated with fat from nowhere! I am a sonographer and have adjusted my gray scale on my screen with my liver for years. just one year before ketek my liver was completely normal.
    I live in NC and my doctor is one of the transplant specialist at
    CMC. I think a lot more shall be revealed about this drug.

    I appreciate you sharing your information with us, as we are monitoring the Ketek situation.
    As you probably know, the European Medicines Agency (EMEA) said in a January 27, 2006 statement that it had asked Sanofi-Aventis to include stronger warnings on Ketek about potential liver problems. For more on this development:

    Isn’t it true that all antibiotics have caused liver dysfunction, including rare cases of death?
    Why would Ketek be any different than say, Augmentin, Avalox, or Zithromax?
    I understand the “better safe than sorry” mentality, because as Cathy above can attest, problems taking medications are a very real possibility. But shouldn’t every other antibiotic be held to as high a standard as Ketek is now being held to?

    I just took my last dose of Ketek today (2/18/06). It was prescribed for sinus, upper resp. problems, fluid in ears, sore throat..etc.
    Today I decided to do research on the drug since I knew it was new because I had never heard of it. I was shocked to hear of recent liver failure due to the drug. How long after taking the drug before liver failure symptoms appear? I’ve noticed this antibiotic doesn’t seem quite as quick to relieve my symptoms as other drugs I have taken. Maybe my infection is not bacterial, if that’s the case then the drug shouldn’t work at all. I have noticed while taking this medication a decrese in appetite, and some weight loss. My main concern is how long after finishing the medication should I watch for side effect symptoms.

    It is my understanding that, like most drug injuries, Ketek can harm the liver only during the period when the active chemical of the Ketek (telithromycin) is still in your system, i.e., shortly after you have stopped taking the Ketek. This is not a situation where you need any “medical monitoring” for some delayed, or latent, reaction sometime in the distant future.

    My grandmother took Ketek for a sinus infection for approximately 10 days in June 2006. Although elderly, she was very healthy prior to taking the drug, with no signs of liver damage. Shortly after she began taking Ketek she developed jaundice, and her liver tests indicated problems. The doctors were puzzled as to what was causing this, and they thought they were going to lose her. Finally she decided on her own to stop taking the medication, and the symptoms have cleared up. I don’t know if any permanent damage was done. This is a dangerous drug. Whoever says the benefits outweigh the risks didn’t see the pain my grandmother was in.

    My son contracted drug-induced hepatitis after taking one dose of Ketek for a minor sinus infection. We’re talking about a healthy, athletic 18-year-old that lost 20 of his 190 pounds in 12 days and stayed jaundiced for two months after taking one dose. He’s lucky he didn’t take a second dose – he’ll recover. This medicine needs to be pulled off the pharmacy and pharmaceutical sample shelves and re-evaluated for safety.

    I was put on Ketek for lyme disease after the Dr tested my blood to make sure my liver studies were normal and there were no previous problems. i was on ketek for 8 weeks , It worked well in taking away the lyme symptoms.But i started to feel naseated -did blood test and my liver enzymes were very elevated . I stopped the ketek.Started taking milk thistle recommende by the DR.After 4 weeks-liver enzymes were still elevated-no change.I went on a liver cleansing diet by dr sandra cabot and in 6 weeks my liver enzymes were noraml.4 months later im having problems-itching skin and swelling of my ankles-so i will be having a biopsy done to find out the diagnosis.

    My husband was given Ketek for a sinus infection in January. Thirty minutes after taking his first dose we ended up in the ER and he almost died. He later started having severe stomach pain and the doctors said it was just a common stomach virus. Well after applying for life insurance and they required a physical with a full blood panel we were informed that his liver function is elevated. So we are now in the process of finding out if he suffered liver damage as well in January and over looked by doctors till now….
    So You can not say that Ketek will only effect you while you are taking it. Cause the damage can be done and not found for several days or months…
    BEWARE OF KETEK…..

    I took Ketek for a sinus infection. Two weeks later I noticed I was yellow and had slight discomfort on my right side. My doctor did some blood work on me and wants to go over the results with me tonight. I am scared. I only took Ketek for 5 days. Can the liver get better on its own just after on time on Ketek? Please if anyone can tell me I would so love to know. Thank you.
