Blood Thinner Medication Warfarin Linked To Brain Bleeds In Elderly

Patients More Than 80 Years Old At An Increased Risk Of Intracerebral Hemorrhage

The January 9, 2007 edition of the medical journal Neurology has a report concerning a new study which reveals that elderly patients using the blood-thinning medicine warfarin may be at greater risk for having a serious brain hemorrhage.  Warfarin is sold under various different brand names, with one of the more popular being Coumadin.

According to a January 9, 2007 Newsday article by staff writer Jamie Talan about this Neurology article regarding the new warfarin study:

Scientists at the University of Cincinnati College of Medicine found that use of the anticoagulant medicine increased in the 1990s, and the surge in prescriptions caused a rise in the number of drug-induced intracerebral hemorrhages, especially in people over age 80.

"We’ve had no idea how often this was happening," said Dr. Matthew Flaherty, lead author of the study that appears in the journal Neurology. Use of the blood thinner increased after many studies showed warfarin was effective at preventing ischemic strokes in people with atrial fibrillation, an abnormal heart rhythm.

As background, ischemic strokes are caused by a blood clot that forms in the brain or travels to the brain.  By contrast, intracerebral hemorrhages involve a blood vessel bursting in the brain; as such, it is commonly referred to as a brain bleed. An intracerebral brain hemorrhage is a relatively rare form of stroke, accounting for less than ten percent of total annual stroke events.

The practical effect of this news about the increased risk of brain bleeds in elderly patients using warfarin, or Coumadin, was covered in a January 8, 2007 article by HealthDay reporter Steven Reinberg:

One expert thinks that doctors need to evaluate a patient’s risk of stroke versus their risk of bleeding before prescribing warfarin.

"This study demonstrates that we need to be careful when we use these therapies," said Dr. Michael B. Rothberg, an associate professor of medicine at Tufts University School of Medicine.

Not all patients with atrial fibrillation will benefit from warfarin, Rothberg added. "Not all patients with atrial fibrillation should be getting warfarin," he said. "Patients at the highest risk for stroke will benefit the most, and patients at the highest risk for bleeding will benefit the least," he said.

Rothberg noted that although warfarin is standard treatment for atrial fibrillation, not everyone with atrial fibrillation is at the same risk of stroke. "I don’t think that most doctors prescribing warfarin are assessing their patient’s risk of stroke and risk of bleeding, but they should be," he said.

In closing, we want to make an important point:  As with all prescription medications, patients should talk to their doctors before making any decisions not to take warfarin, or Coumadin.

    I WAS READING THIS ARTICLE AND WAS PUZZLED TO FIND THAT AM NOW IN A POSITION LIKE THIS I JUST LOST MY DAUGHTER JANUARY 12TH SHE WAS IN A KAISER HOSPITAL IN CALIFORNIA SHE WAS ISSUED POTTASUM ALONG WITH BLOOD THINNER I THINK IT WARFARIN SHE THEN HAD SEIZURES AND DIE I NEED SOME ANSWERS AS TO WHAT IS THE SIDE EFFECT OF BLOOD THINNER AND POTTASUM AS OF NOW THEIR IS STILL NO CAUSE OF DEATH BECAUSE A DEATH CERTIFICATE WAS NOT ISSUED TO ME ALL OPINION IS APPRECIATED THANK YOU.

    I just have one question about the coumadin (Warfarin). Can it be taken with the Flax Seed Oil if a person is taking the coumadins (Wafarin)?

    Unfortunately, I don’t have the answer off-hand. I suggest that you use one of the online drug interaction checkers — like the one found under the Resources section in the sidebar of this page. Better yet, you should speak with the doctor prescribing the warfarin and/or the pharmacist filling that prescription.

    I am seeking ANY information about a new Clinical Trial Drug that takes the place of Warfarin. I think it is called Bibigatran (??) Not sure if have the right spelling. I would appreciate any and all info on the trials, where are they being done. Is it a better medication that Warfarin? Is the VA doing them, My husband is a Vet and goes to VA. Thank you.

    I was wondering if you knew of any protective effect against stroke in patients taking warfarin along with hormone replacement. If someone with afib is taking warfarin do they still have the increased chance of stroke associated with hormone replacement?

    my grandmother was taking this particular medication. she overall was very healthy besides the extreme headaches resulting in a blood clot on the brain. she died december 4, 2007. her death certificate came back as massive stroke on brain and brain hemorrhage. shes being taking this medication since 1986. Is that too long for a person to be taking blood thinners. And do you think that warfarin was the result of her death.

    I am sorry to hear about your loss but please know that we appreciate you sharing your concerns with us.
    As I am not a doctor I am not able to give a medical opinion about whether the brain hemorrhage is related to the Coumadin (warfarin) or not.
    I will point out that the medical journal article regarding the possible association between brain hemorrhages and Coumadin (warfarin) use is recent. As such, most doctors are probably still unaware of the possible problem — if, in fact, there is one, here.

    I was put on Coumadin after pacemaker placement. I then had a hemorragh in my eye resulting in permanent damage and partial loss of sight – necessitating several operations on my eye. Has this happened with Coumadin.

    As an attorney and not a doctor, I am not in a position to tell you one way or another whether your eye problem is a Coumadin side effect. You should address this issue to one of your doctors.

    Im still reeling from my dads death at age 65 from warfarin causing a clot and then brain vessel pop leading to bleed and fluid build up and death. Im trying to find info and i see all the people having family members die while on this drug! I feel my dad didnt realize HOW dangerous the drug could be for him because it is different for all. He did have to much in his system to clot the bleed.

    I have so much to say about this drug.frist and formost my brother died in june of this year he was 53.they put him on warfain he said he felt funny when he took the drug.the day he died he said his head was throbbing like a bad tooth .he laied his head down to see if it would pass and he died.they didnt do a autopsy said he had a heart attack but he never complained not one bit about any chest pain.in the mean time my husband was puton this same drug .he passed out on me twice he had heart valve replace ment the surgen told my husband if he took pigs valve he wouldnt have to take the med no more so thats what he did.Now in the meen time my husband has atrilfibulation.and the Dr calls and wants to put him back on the warfarin.we told his nurse he wanted to talk to the docter.while wating to go back to see the doctor my husband starts forgetting where hes been what he has done talking about his mother that has been dead for twenty years but tells me to call her.when we go to the doctor he get mad because we questioned the drug.two days later we get a letter stating he vwont treat my husband any longer.my husband is not healed from his heart surgery I am scared and upset that a doctor can do this just because we had questions abut the drug.everyone we have talked to that had a family member that took it died what would you think.thanks for letting me tell what has happen

    I think the hemorrhage stroke is related to the warfarin. My mother was taking it for 6months. Out of nowhere she had a massive stroke & died 4 days later.

    My father was on warfarin and passed away. No one cared and gave me many excuses that it was natural death he was old and he lived a long life. I saw my father suffer for a real long time and I knew it was because of his medication . My father was prescribed alloperinoal and had a bed redaction that caused Steven Johnson syndrome.. I saw him go through that. And again no one would take his cause . Now this again . I need help to have my fathers voices heard. He mattered to me and was a human.
