Oral Anti-Fungal Lamisil Associated With Liver Failure And Other Hepatic Adverse Reactions

February 2008 Australian Report Links Liver Failure And Hepatitis To Popular Novartis Drug

(Posted by Tom Lamb at DrugInjuryWatch.com)

A February 4, 2008 article, "Anti-fungal tablet linked to liver deaths", published by Australia’s Herald Sun newspaper, brought to our attention the fact that Australian drug regulators have issued a warning about serious liver reactions after taking a popular prescription drug used to treat fungal infections, Lamisil (terbinafine), which is made by Novartis Pharmaceuticals Corporation (NVS).

We get an overview of this Australian regulatory report from the February 4 Herald Sun article:

The Therapeutic Goods Administration (TGA) has issued a warning about serious adverse side effects reported with oral Lamisil, a pill formulation for ringworm and nail fungal problems.

The medication is commonly prescribed to people who do not respond to topical fungal creams, but the regulator’s Adverse Drug Reactions Advisory Committee (ADRAC) warns it can cause liver failure.

The committee has received 722 adverse event reports related to Lamisil, known generically as terbinafine, including 70 liver reactions, 61 implicating the tablet form as the sole suspected drug.

Those affected ranged from 20 to 85 years old, with half suffering their liver reaction within the first month of taking the pills.

For more details, we take this excerpt from the ADRAC report about Lamisil, "Hepatic reactions with terbinafine", which appeared in the February 2008 Australian Adverse Drug Reactions Bulletin (Volume 27, Number 1):

Of the total 722 adverse event reports received up to January 2008 in connection with terbinafine (all dose forms), 70 describe hepatic reactions and most (61) implicated oral terbinafine as the sole suspected drug. Onychomycosis was the most commonly cited reason for use of terbinafine; patient age ranged from 20 to 85 (median 58) years, and men and women were affected equally. Half of the reports documented onset of hepatic reaction within the first month and 80% within 7 weeks. Most of the reports document minor abnormalities of liver function but 3 describe fatal liver failure, 10 describe hepatitis, and 12 describe jaundice. Full recovery was noted in 27 reports but 34 cases had not recovered and the outcome remained unknown in 9.

This same report pointed out that "ADRAC has previously drawn attention to serious adverse reactions associated with orally administered terbinafine", specifically:

Returning to the the Herald Sun article for a report of the response from Novartis:

A spokesman for the drug’s manufacturer, Novartis, said that serious and life-threatening liver reactions were rare and well documented side-effects of oral anti-fungal medications.

The company said it agreed with the advice issued by ADRAC.

In the November 2005 package insert for Lamisil, under the WARNINGS section, one finds the statement "Rare cases of liver failure, some leading to death or liver transplant, have occurred…."  There is a similar mention in the ADVERSE REACTIONS section of the current Lamisil package insert, or label.

We’ll watch to see whether this regulatory alert about serious liver side effects prompts any Lamisil label change by Novartis in Australia, at least.

My husband was prescribed this drug a few years ago and took it for almost three years. He is now suffering from a ‘fatty liver’ and other associated problems. His GP prescribed the drug for a fungal problem that did not go – in the last year our GP has retired early due to an alcoholic problem – I hope that my husband will recover and I can keep you informed. Professionally I am involved in the area of Health and Safety and am aware of the toxic properties of drugs and their cumulative affects.

Thanks for sharing this information about an apparent side effect due to Lamisil use.
If you get any confirmation that the fatty liver condition was caused by the Lamisil, you can let us and others know by submitting another Comment, here.
Thanks for reading Drug Injury Watch.

Fatty liver is far more likely to be due to excess intake of alcohol or carbohydrates, especially fructose. It is very common in pre-diabetes and type 2 diabetes.

I took lamisil about 5 years ago….and it is the only prescription drug I have taken or been prescribed in the past 12 years. I took it for several months and it did not clear up my toenail fungus (although vics vaporub did). I have suffered from digestive problems due to liver problems ever since. It wiped out all good bacteria in my intestines and has made several foods and additives intolerable. I wrote at formal complaint to the fda several years ago. It is labeled one of the most dangerous prescription drugs in several medical journals and books.

Thank you for sharing your experience using Lamisil.
Out of curiosity, can you describe for us how you used the vaporub to clear up the toenail fungus, e.g., how often applied, period of time you used until fungus improved or eliminated, etc.
    Thanks in advance.
    Have a good day.

I got acute pancreatitis twice, once two years ago, and again yesterday (I am in the hospital right now recovering) WITHIN ONE WEEK OF STARTING LAMISIL. I AM NOT AN ALCOHOLIC (which is another cause) although I do consume alcoholic beverages occasionally on weekend date nights with my husband = a total of 3-4 drinks per week some weeks and “0” zero others. I do not take any other Rx drugs besides Prilosec and Allegra, but have never gotten pancreatitis before or after- only within one week of starting the Lamisil, and IMMEDIATELY went off of it after I got the Dx. The first time I only suspected it, so when I went on Lamisil the 2nd time, I was worried, and sure enough it happened again! NEVER TAKING THIS MED AGAIN-IT’S DANGEROUS AND SHOULD BE RECALLED.

I just got out of the hospital with pancreatitus from taking the generic for Lamisil – Terbinafine. I was only the drug for about 2 weeks when I had a severe attack and was hospitalized for 3 days. This was my third pancreatic attack in the last two years and alcohol was blamed but I do not drink. A CT scan was taken and only inflammation showed. No stones or masses or tumors so it has to be the drug. That was the only other thing introduced to my body.

My mother was prescribed a Lamisil for nail fungus about 10 years ago. She had her enzymes checked regularly, and went off of it when they became elevated. She was diagnosed with terminal liver cancer in 2008 and died 90 days later. She had terrible digestive and intestinal problems after the lamisil, up to her death. She herself wondered if the lamisil was to blame for her cancer.
I would caution against taking this drug. The promise of sandal-perfect toenails is not worth the risk.

    Monica in Mass,
I just got back from Puerto Rico, my dad has liver cirossis. He’s not an alcoholic I have never seen him drunk or buzzed. But I remembered him taking meds for nail fungus and I asked my sister was he still drinking it. Mind you he told me over a yr ago that he was drinking this. My sister replied yes for about 2 yrs. my jaw dropped in horror. Cause I know you can drink it that for that long. His sin was not following up regularly with a doctor. Cause they could have stopped it. We are waiting results from a liver specialists. But he’s a 63 yr old healthy male that sadly did this to himself unknowingly. I have fungus on my pinky so I knew the possible side affects and am currently using Vicks vapor rub. Hopefully it works. My dad, I’m hoping there is something we can do.

I have been on Lamisil for 7 weeks. I’m suppose to take it for another 5 weeks. I will never take this medication again. I have so much anxiety, panic, angry, depressed feelings.I have stuck it out as long as I can. No I just pray that my toenails continue to grow out the rest of the way nicely. I would rather have fungus toenails than to Continue this medication.

I see no one has written on here for quite some time. But I also was on lamisil for almost a year.. I just turned 30 yrs old and have never had any serious health concerns. But I started to get horrible pains while on this medication . My gp and gi doctor kept saying I was fine. So one sat in the middle of the might, woke up and was vomiting nonstop . But my doctor s said I was fine. So I figured it was an unbearable stomach virus .But it wouldnt stop and by the 3rd day I had not been able to keep anything down and so dehydrated I went to the hospital. It turned out that I had extreme pancreatitis. So I was admitted and strictly iv fluids only for 6 days because my pancreas was so inflamed
. They ran tests and said I had to stay another 4 days and have my gallbladder removed. So I thoughtafter all of that excruciating pain and having surgery I’d start to feel better. But I was hospitalized 3 more times w pancreatitis .I don’t drink often. Maybe a few beers in a weekend. I’m wondering if anyone else who had pancreatitis feel better and back to their normal self, or if this is going to be an ongoing problem.. any feedback would be appreciated.

I was on lamisil for about a year. . I just turned 30 and have never had any serious health concerns until I stopped the medication . I was hospitalized with pancreatitis .the pain was unbearable.. They ran tests and said I had to stay at the hospital for 7 days on strictly iv fluids. Nothing to drink or eat. So I finally had the surgery and thought finally I’d start to feel better . Boy was I wrong . I have been hospitalized 3 more times with pancreatitis. I’m wondering if anyone else that had this issue felt better. Or if this is a life long issue I’ll have to deal with . All over a pill that d idnt even work.. And the hospital refused to give me my thyroid and anxiety medicine and I started having seizures when I got home. Is there anything I can do about the way they treated me and I even had my medicines with me and they said don’t worry we have them here. Well I ended up in psychosis because they just stopped my anxiety medicine that I was supposed to take 4 times a day.. I’m really having a tough time dealing with all of this and ho ping someone may know if the pancreatitis will stop. And also about them refusing to give me my merication..

I took lamisil for about 6 months, 10 years ago. It cleared up a couple of toes and improved one. I had to stop taking because I lost all taste. A few weeks after stopping my taste came back. I still have intestinal problems in the area of liver. My gall bladder has been removed. I’m wonding if this could still be affecting my life.

I have had serious digestive problems and vomiting since been on Lamisil. Vomiting bile. Vicks vapor rub helps the toenail fungus, applied three times daily and wear a footie or sock on feet at bedtime, but it hasn’t cured it.
