Teva’s Budeprion XL Will Get Another Safety And Efficacy Review By FDA

FDA To Collaborate With Complaining Patients On Study Of Reported Problems With This Generic Version Of Antidepressant Wellbutrin XL

By means of a September 23, 2008 Wall Street Journal (WSJ) article, "Wellbutrin Generic to Get New Review", we learned that the FDA is going to re-visit the issues of whether Budeprion XL — which is made by Impax Laboratories Inc. and distributed by Teva Pharmaceutical Industries Ltd. — is as safe and effective as Wellbutrin XL — made by Canada's Biovail Corp. and sold by GlaxoSmithKline PLC.

From the September 23 WSJ article by Andy Georgiades:

Five months after declaring a generic version of antidepressant Wellbutrin XL safe, the U.S. Food and Drug Administration says it is prepared to conduct a human trial to get to the bottom of consumer complaints about the drug….

More than 100 complaints about the drug were filed with the FDA between December 2006 and the end of this January. Among the most common complaints were that the drug was ineffective or aggravated the condition, or caused nausea, anxiety or headache.

This past summer, [Joe Graedon, a pharmacologist and co-founder of the People's Pharmacy] and others were granted a meeting with senior officials from the FDA's Center for Drug Evaluation and Research.

Mr. Graedon said there was a "consensus" by the end of the meeting that the two sides could collaborate on a study that would involve patients who reported problems with the generic after taking the branded version. But he can't begin a recruitment effort until he is given the details of the study design, and he is still waiting to hear from the FDA on that.

This September 2008 WSJ article about generic Wellbutrin reminded me of a man who contacted me in mid-August 2008 regarding an "extreme reaction" — he said the ultimate diagnosis was septic shock — which he had within an hour of ingesting his first dose of Budeprion XL.  Taken to the ER, he was put into the Intensive Care Unit (ICU) for several days, and was hospitalized for more than a week until his condition stabilized.

After taking Wellbutrin XL for years without any problem, this man could not understand why Budeprion, which was "only" the generic version of Wellbutrin, would produce this suspected (by his doctors) serious side effect.

Then he found online this article, "Are Generic Drugs Safe?", by Joe Graedon, MS, and Teresa Graedon, PhD; it was published originally in the February 2008 edition of Prevention magazine, which he forwarded to me.

In relevant part, from this February 2008 Preventionarticle by the Graedons:

On March 20, 2007, we contacted the FDA with our concerns; both Robert J. Temple, MD, director of the Office of Medical Policy at the Center for Drug Evaluation and Research, and Gary J. Buehler, RPh, director of the Office of Generic Drugs, told us that the agency would look into it. But the FDA moves slowly, and we were anxious to get to the bottom of this, so we teamed up with ConsumerLab.com to take a close look at Wellbutrin XL 300 and its equivalent, Budeprion XL 300, as well as two generics for sustained-release Wellbutrin. (Most generics go by the name of the active ingredient–in this case, bupropion hydrochloride. But some, like Budeprion, are given distinctive names.) ConsumerLab.com bought samples of each drug to test, using two labs to double-check the results.

You can read about what was found-out by the ConsumerLab.com testing team when you read the Prevention article, which I recommend if you are truly interested in this subject.

We will watch for more about the FDA study comparing Budeprion to Wellbutrin in terms of safety and efficacy, as well as continue to monitor other emerging generic drug issues such as quality control, good manufacturing practices (GMP), and the like.

It seems — from Actavis Digitek pills and Ethex morphine pills being double-the-intended-dose in some instances, to this current alleged problem with Budeprion — we should not blindly accepted the "pitch" from health insurance companies that the generic drugs they want you to take, now, are just as good as the brand name medications that they switched you from for cost reasons.

    My prescription was for Welbutrin and I was given BUDEPRION as the generic drug. I have been taking it for several weeks. I have had increasing depression during this time which prompted me to look it up to see if it was the same as Welbutrin. Believe me it is not. Beside the fact that it does not help my depression, I have had increasing sleeplessness, waking at 2 or 3 AM and not being able to go back to sleep.

    About a year ago, my pharmacist substituted the generic budeprion for my Wellbutrin XL. It was awful. I was anxious, disoriented, and really down in the dumps. The Wellbutrin is three times the cost, but my sanity well worth the price.

    I couldn’t agree more, I’m going on my 3rd week of Budeprion XL .(After great success with Cymbalta, but a change in Insurance companies made this too expensive.) I have never felt so horrible in my life. I have shortness of breath, constant lethargy and most of the time it feels like there is a Vice-Grip on my head just above the ears. I am researching right now to find something else. Is Bupropion a better match with Wellbutrin ?

    I have been taking taking budeprion xl for about four months now with no notical problems at all and in fact my anger, constantly feeling stressed out and mild depression have nearly disappeared! Now mind you, ive never taken wellbutrin ever so that could be the difference in my case?

    I started taking Budeprion XL about 2-1/2 weeks ago. I have noticed TERRIBLE ringing in my ears so intense that it makes me dizzy, nausea, increased anxiety, and inability to stay asleep. Two years ago, I was on buproprion as a generic for Wellbutrin XL with no problems. I’m trying to find a pharmacy that carries bupropion that is NOT Budeprion.

    Mr. Lamb,
    Has the FDA done this study yet ? I too experienced horrible side effects from the generic budeprion, including blurry vision and a host of other physical problems resulting in an MRI, blood tests and Opthamological testing. The vision damage is permanent. I’d like to know if there has been any FDA involvement since these posts last Fall 2008.

    My daughter had been taking Wellbutrin XL for about two years, but had experienced increasing side effects, the most prominent of which was what he called “overheating”. She would initially perspire heavily, then “dry out”. She would then feel too hot, nauseated, and go on to develop severe headaches, like the ones described above — a band tightly wrapped around her head. Physical activity became more and more limited as she feared the overheating. Her new doctor switched her to the generic Budeprion XL, and the symptoms got much worse. The drug was removed, and she feels much better.
    I am a therapist, and I attended a conference recently on SSRIs, SNRIs and suicidality. The presenter’s opinion, after 35 years in practice as a psychiatrist and psychopharmacologist, was that the real reason for the increase in suicidal ideation and risk of suicide attempts following the initiation of SSRI and SNRI therapies was the side effects. The patient, already feeling hopeless, helpless and physically compromised, is told he will feel better on the medication, only to experience side effects that are annoying at best, and devastating at worst.

    I have been taking budeprion xl for a week now and I have been having ALOT more trouble sleeping than I did before and I have insomnia so that says something. I have also been having stomach troubles where it hurts so bad like I have to vomit but I just can’t and then I have bowel movements that take forever to happen. And to top it off the medication doesn’t even seem to be working for me anymore. The first day or two I thought it was but now everything just seems to aggravate me faster than before. I am just being paranoid or have other people found the same side effects happening to them?

    was taking wellbutrin xl four years ago… and feeling better after 2 years. then switched to bupropion xl, due to insurance co. right to substitute, didn’t feel as good, but better than when i was diagnosed with bi-polar disorder and not medicated. now have been switched to budeprion xl, again, it’s cheaper than bupropion xl. i suspect that next i’ll be give asprin (generic). does the F.D.A. really care though? pharmacutical companies; big companies, big money… strong lobbiest. insurance companies; big companies, big money… strong lobbiest. us, we’re at their mercy; we have no choice in the matter. it took many years to come up with the “right” combinations of chemicals and create wellbutrin; which really worked for me and so many others. how is it, now they can “kinda” make something similar and expect it be be as effective and safe? they don’t it’s collateral dammage… after all, no one will realy, or just won’t, admitt that the “side-effects” are due to the medication. many doctors are in on this too. i have been put on so many medications, given samples (to the point i felt that they were working) only to get a generic once the samples were gone… some doctors chart your progress and side effects when you are taking certain medications. they report these to the drug companies… a “stipend” reimbursment (for their time) gifts and “publishing rights”. you’ve gotta publish if you want to climb the ladder of corporate medicine. good luck to all of us out there; relying on the care, products and services of the corpoate entities which have us dangeling by a thread.

    I have taken Wellbutrin….the generic Buproprian actually, with NO side effects for 3 years for Adult ADD. I thought I was getting the flu last week, I was clammy, dizzy, my brain felt like it was floating inside my head and my eyelids felt like they had lead in them. I slept most of the day because I was too tired to stand and when I did sleep I had the most bizarre dreams that I’ve ever had in my life. After two days of this, I began to have chest pain and got very frightened (I’m healthy, active and have never had heart issues.) I tried to figure out what might be happening when I remembered my Express Scripts had sent me budeprion SR instead of my usual Buproprian. Just out of curiousity I searched it and was HORRIFIED to learn of the complications this “substitution” has created in perfectly healthy people. I called Express Scripts and spoke to a pharmacist who said I was having a bad reaction to the drug after cross checking the manufacturer with what was printed on my pill. She told me to stop taking it immediately and she would call my doctor to get approval for a substitution. Within 24 hours of stopping this drug I felt normal again. My question is…with all these concerns how long will it take until the distribution of this drug is halted? Will people have to die first? On the pharmacist’s recommendation, I have filed a report with the FDA. It was a very frightening experience. For those of you still having reactions, you sometimes CAN get your original prescriptions back if your MD specifically writes an order for it. Express Scripts said they would honor that.

    This is quite amazing. I thought that I was going crazy for the past 3 weeks. My doctor prescribed Wellbutrin SR 100 mg, three times per day. The pharmacy gave me bupropion. I had no problems what so ever until my doctor switched me to Wellbutrin SR 150 mg, 2 times per day. The pharmacy gave me the budeprion this time around. It didn’t take long for me to lose my energy, become sensitive, irritable, my eye sight blurry and definitely not myself. I contacted my pharmacist and she insisted that the medication is the same. However, I contacted Teva as well as Glaxo and the medication might have the same active ingredients but the inactive ingredients are different. I can’t handle being on budeprion so I contacted my insurance company, Oxford-United Health. They refused to lower my copay if I wanted Wellbutrin because the generic version is available. They don’t understand how much it’s affecting me!
    The budeprion costs me about 11 dollars for one month supply while the Wellbutrin will cost me 167 a month. I can’t afford to pay my insurance on a monthly basis and also have a presription costing me so much money. Oxford’s appeal department has not bothered to get back to me with regards to a lower copay and the expedited claims department at Oxford told me that there is nothing that they can do. So I either stay on the budeprion and end up costing the insurance company more money down the road because of their negligence or I keep on fighting. I finally went to a different pharmacist and the pharmacist told me that she was going to contact my doctor so he could give me the original prescription, which is bupropion (still generic, but I never had any issues). At least this one is recognized by Glaxo whereas the consumer rep at Glaxo had no idea what budeprion was! The pharmacist was kind enough to give me an emergency prescription advance for bupropion- same one I had before. Unfortunately, it’s now the weekend and I have to wait til Monday to get a different script from my doc. I hope I make sense to some of you who have had to go through what I did or are currently experiencing a change in your depression due to the budeprion. This is quite scary and I wish there was something legal that I could do to stop these insurance companies from putting our lives at risk so they can save some money on generics. Any advice? Thanks.

    I had taken Wellbutrin Xl for quite a long time about four years ago. I had remarkable success with it. And then, like many who have presented their stories, I was switched to Teva’s generic. It was rather surprising how rapidly the changes occurred: I began having panic attacks, which I’d never had before; my moods shifted wildly; and I was tense more often than not.
    It didn’t make sense to me, and I was switched to Cymbalta (different story altogether with that one). But I couldn’t figure out what had caused the shift, especially with the FDA, insurance companies, and my doctor all saying the generic worked similarly to the brand name.
    Once I read about the Teva investigation by ConsumerLabs, I knew what had actually occurred. I initially thought Wellbutrin had simply stopped working, and I needed to switch, but my experience seems awfully familiar to that of others.
    I am currently without an antidepressant, and making by (for the most part). A new doctor prescribed Wellbutrin XL again, which I’m hesitant to get, because it’s $85 for the brand name ($10 for the Teva generic). That’s a hefty $75 difference, but when it comes down to it, I will pay that difference if I have to. I refuse to take Teva’s generic until the FDA mandates a correction in their production of it.

    I have been on Bupropion HCL 150 mg nearly two weeks. I have noticed irritability, dizziness, headaches, upset stomach, uncontrollable crying and just a really crappy feeling.

    After After10 years on Wellbutrin, I was switched to a generic, budeprion made by Teva generic drug company. Approximately 4 weeks on the generic my stable, non-eventful depression was turned upside down, nearly to the symptom-level suffered when first diagnosed with depression. My sleep is intermittent at best; headaches, abdominal pain, and some joint pains have occurred while on this generic budeprion. I discussed these issues extensively with the prescribing physician (who ordered ativan – also generic – to off-set the side effects of budeprion), and with the pharmacist. Neither can help because they have no control over the insurance companys’ policies. Now I take 2 generic medications in place of one branded; the two combined are not helpful, they serve only to add to my depression. Where can one seek information to correct the wrongs imposed by the generic drug company and the insurance company? Pharmacists and physicians voice’s should be heard to ensure patients best interests while on the products they prescribe, or act to make their patients’ concerns known to industry and the FDA.

    Child on this generic medication as BC BS will only cover generic…no stability as with real wellbutrin and it is $65 for a generic..she is 18 and needs to have real stuff…pretty sad that I will have to shop a canadian pharmacy to find real thing for her to have relief after just paying a dr’s copay and the $65 copay for this terrible generic…and did we mention…we are both unemployed…thanks for this information as we thought it was just her….

    I started taking Budeprion (Wellbutrin XL 300mg) a couple of years ago. It did take some getting used to, but I seemed to level out. This past January, when I had my RX refilled, I was given Buproprion & was told by the pharmacist it was “the exact same thing”. It was NOT! I gave it 3 months of full refills -couldn’t get replacement – and was miserable. I had headaches, weight gain, insomnia, and was really quick to get aggitated. I requested my pharmacy change back to the TEVA brand and have had to jump through hoops every month since. I have been told the pharmacy doesn’t order from that company anymore and that TEVA charged them $1000.00 per bottle (which I doubt). A co-worker & friend of mine was prescribed Wellbutrin XL & went through the same thing. However, she started out on the Buproprion. She constantly had headaches, backaches, and just felt bad overall. I told her about the TEVA brand and she had had to find a pharmacy to get the Budeprion, too, but has been much happier since getting the brand switched. I wonder if it just has something to do with individual hormone balance and/or molecular structure (I am obviously no Dr.). I make sure to stay hydrated – drink a lot of water, because I do notice that if I get a little “dry” my jaw hurts and I am more easily frustrated & less able to tolerate aggrevation. I plan to stay on the TEVA brand and will switch pharmacies if I have to if my pharmacy can’t get me the right brand.

    This stuff is just flat out CRAP. Don’t do it!!! If you’re on Medical or on the County and therefore “sentenced” to it, get a “Tarr” (override) form from your doctor for the real Wellbutrin or switch to something else, although if Wellbutrin works for you as well as it has worked for me for the last 15 years, there’s no substitute…. I do hope that those of you who are getting faked out with this garbage at least aren’t having to pay for it!?!

    I had never been on any medication before, so when I was prescribed the Budeprion for some mild situational depression, I had no idea what to expect. I became a totally different person. My mild depression turned to suicidal thoughts, paranoia, obsessiveness, lack of concentration, continual crying and a hair-trigger temper. I would sit on my couch and think of ways to kill myself…..and had NEVER had a suicidal thought IN MY LIFE up to that point. I lost all interest in EVERYTHING, and was certain that my husband and 6 month old son would be better off without me. I seriously thought I was going crazy. After looking up Budeprion online and seeing what everyone was saying about the side effects, I had my Dr. put me on name-brand Wellbutrin. Almost instantly I was back to being a normal person again! No teariness, paranoia, suicidal thoughts, etc. If I had stayed on the generic medication, I most certainly would’ve jumped off of a bridge by now. The result of continued use would’ve been catastrophic.

    For years I was switched from one antidepressant to another. Finally I was put on Wellbutrin. I got some Anchen product which, after several months, I stopped taking because it made no difference at all. Truthfully, I didn’t feel any worse for not taking it. I tried bupropion again with another doctor, and I happened to get the Teva product. WONDERFUL! I have been specifically asking for this brand for the last 8-9 months at the pharmacy and have been doing very well on it. Now this is going to end and I have to go back on the stuff that doesn’t work?

    I was on the Brand name Wellbutrin SR for 5 years. Then I had to change because of my insurance that will only pay for the generic even with the Doctor writing Brand Necassary on my prescription. I was taking Bupropion and now im on Budeprion. THESE ARE NOT THE SAME AS THE BRAND NAME !! They are both horrible. They do not work! Im depressed,i cry for no reason and my life has no meaning anymore. Drug companies do not care about trying to help people, all they care about is making money. They are too stupid to realize how much money they would make by only selling the brand name at a lower cost. Instead of making a good name for themselves they get to read mail like this. Great way to run a company guys ! Of all the articles i have read it seems like every one would be flushing their Generic pills down the toilet and buying the BRAND name if they could afford it! But no we have to put up with trying to live our shitty depressed life because of ignorant drug companies trying to make a buck. Tell me something, why do you even make the Wellbutrin anymore if no one can buy it?

    I was on bupropion XL by Mylan and got Teva’s budeprion on my last refill. The second day on it I felt stoned and out of it. The third day on it I still felt like I could not even think and started vomiting. I reduced my dose to half until I can see the doctor. I have had an increased appetite on it as well. I did file an FDA complaint.
