Sandoz Metoprolol Succinate ER Tablets Recall Has Been Done Rather Quietly

Many Patients Are Dissatisfied With Lack Of Information From Sandoz, FDA, Or Their Pharmacy About Why This Generic Toprol Pill Was Pulled

(Posted by Tom Lamb at DrugInjuryWatch.com)

In an August 26, 2008 Drug Injury Watch post, "FDA Warns Sandoz About Possible Quality-Control Problems At Its North Carolina Generic Drug Manufacturing Facility", I reported FDA inspectors found that Sandoz had failed to properly validate its manufacturing process for Metoprolol Succinate ER Tablets (Generic Form Of Toprol XL).

Here is the August 12, 2008 FDA Warning Letter (08-ATL-13) sent to Sandoz concerning the"Good Manufacturing Practices" (GMP) problems found at its generic drugs facility located in Wilson, North Carolina.

Returning to my earlier post, there I noted:

It is unclear as of August 26 whether Sandoz will cease distribution of its metoprolol succinate ER tablets (generic Toprol XL) and/or issue a recall of this medication.  We will be watching for these possible related developments.

Well, in the few months that followed we never saw any Metoprolol Succinate ER Tablets recall notice from the FDA nor Sandoz.

Starting in November 2008, however, we started to get some Comments submitted to the August 26 post which indicated that, in fact, some type of recall was underway.  And elsewhere on the web, such as this Topix forum, "recall at Rite-aid — Posted in the Metoprolol (generic), Toprol XL Forum", there were various reports coming in to this effect.

Thankfully, one of the participants in that Topix forum posted a link to this November 12, 2008 notice, "Recall: Metoprolol ER Tablets by Sandoz" published online by Pharmacy Industry News:

Sandoz is voluntarily extending the September 17, 2008, recall of Metoprolol Succinate Extended Release Tablets 25mg and 50mg, to include all strengths of distributed lots with expiration dates through August 2010. This voluntary action by Sandoz is due to deficiencies in documentation practices and in-process controls for this product. This voluntary recall is being conducted down to the retail level with the knowledge of the FDA. The affected lots were shipped approximately between November 2006 and October 2008. (Sandoz Press Release 11/3/08)

Unfortunately (but not surprisingly) I could not find the above-referenced November 3, 2008 press release on the Sandoz web site when I checked today, nor could I find there any press release about this Metoprolol Succinate ER Tablets recall from back in September 2008.

Likewise, there is nothing to be found readily on the FDA's web site about this Sandoz Metoprolol Succinate ER Tablets recall.

If you have more information about how Sandoz is carrying out this relatively "quiet" recall of its Metoprolol Succinate ER Tablets, please let us know by submitting a Comment, below, or sending me a private email.

P.S.  AstraZeneca issued a May 18, 2009 press release to announce that all wholesale ordering restrictions in place for its Toprol-XL (metoprolol succinate) extended release tablets have been removed and that supply levels of Toprol-XL are approaching near usual levels at all points in the distribution chain.  (5/21/09)

I much appreciate you pointing out this December 3, 2008 notice posted on the FDA site.
Based on the September 2008 and November 2008 Sandoz letters regarding the possibly defective or “out of spec” Metoprolol Succinate ER Tablets, one might say it’s better late than never.

I have been taking this drug for over 3 years,an neither the druge store or even the Doctors in my home town have never advise me this drug was on a recall list. The FDA is not doing their job nor is the druge stores or the doctors doing their job.So I am asking you now should i stop taking it now.

  4. Tom Lamb Avatar

    Louis:
I agree with and reiterate this advice from the FDA: You should not stop
taking any prescription drug before talking to your doctor.
I suggest that you call the prescribing doctor and ask his staff whether
your current pill is part of the Metoprolol Succinate ER Tablets recall and,
if so, whether you should be issued a new prescription for a substitute drug
    to take instead.
I hope this information is useful.

    Hi Tom,
Thanks for this website and the information I gained yesterday on Ethex Metoprolol Succinate ER tablets. My husband has not felt well on these tablets that were just mailed via Caremark/CVS on Dec. 11, 2008. So I did some research via you, got him to the doctor today 12/30/08. As I also found others who seemed to have problems with a generic, I went to Ethex/Kv Pharm. website and they had a press release that they have “suspended shipments” of certain drugs, the Metoprolol being one of them. After the doctor’s I promptly contacted Caremark/CVS to see if we could get our money back. To make a long story short, I had to speak with a pharmacist, he knew nothing about the suspension, etc. So I asked him to go to the website http://www.kvpharmacutical.com and look at the press release himself. He was quite taken aback by this news and said he would promptly get this to corportate. He thanked me for the information, and I told him also about your website as well. He also stated we could not get our money back until the drug is ‘recalled’ and we get a call or letter regarding this. But one thing for sure, I know my husband will get better thanks to you, this website and a new drug. Thanks again, Tori

I appreciate the kind words about my Drug Injury Watch blog and this article about the Sandoz Metoprolol Succinate ER Tablets recall, in particular.
It amazes me, still, that pharmacies and pharmacists are not notified FIRST about a recall like this, given that they are the best-last-chance to prevent further distribution and use of pills that may be unsafe for use.
I hope your husband gets to feeling better, soon, and the best to both of you for 2009.
    Thanks for reading.

    Dear Tom,
I have been on Metoprolol succ ER for 30 days now. I received my prescription from CVS with a expiration to discard after 12/02/09.
I had noticed a few days that it seemed the pill did not work which I thought odd because the next day it worked well.
However, I started having problems with urination … then it seemed like a kidney stone only to go away and reoccur. I would have fluid gain and then it go away until finally I am having major problems with this.
ONLY through going to google to see if this prescription could have ANYTHING to do with my problems did I find this medication was recalled!
I found documentation in other countries list difficulty with Urination with possible UTI and bladder infections because of the side effect yet I really found none with entries from the U.S.
After finding this out – I will be visiting the ER today to see if there is anything they can do about the complications and IF they are truly related to this medication.
Even if not – I can still tell you there were days I felt the medication worked and days it didn’t.
Keep up the good work – I would love to have you on my radio show sometime.

I too was given this generic abount a ago at walgreens. I take Toporal XL for an arrythmia (SVT’s) and within 24hrs of taking the sandoz I was having chest pain and svt’s so bad that it triggered a bard migraine. I went to the Walgreens manager and explained that it was not an equal substituation and could be dangerous to those taking the medication since it did not work and caused strange side effects. Since then every now and then they tried to provide me with thex which I refused and requested the Par Manufacture. The have ordered the generic PAR strictly for me since I refused to take this drug. It is interesting to read now what I experienced a year ago. I attempted to pick up my refill and was told NO generics were available until Feb claiming backorder? This lead me to run a search on why and here I am. Best of luck to all that have been affected by the half regulated perscripts we have to take. Stay safe, Heather

I appreciate you sharing with us what you have been through with these generic drugs — where the pills, as you have learned, may or may not have been what was specified by your doctor’s prescription.
It seems to me rather remarkable in this day and age that there are drug companies such as Actavis / Mylan, ETHEX / KV Pharmceuticals, and Sandoz / Novartis having recurrent, systemic problems with what the FDA calls “current Good Manufacturing Practices” (cGMP).

    Hello,
And thanks for making this available.
I began taking metoprolol last spring for an irregular heartbeat. While it seemed to help I noticed during my second month of treatment, the tablet size was noticeably larger. I was experiencing side effects like a heavy metallic taste on my tongue, stupor, intense skin crawling sensations and my teeth hurt. I told the pharmacist and my doctor and was assured it was the same dose size and was safe. As I refilled my prescription sometimes I’d get the large oval pill sometimes a smaller round one but every time I got the larger oval pill I was sure these symptoms worsened. I brought this to the attention of my doctor and pharmacist and the pharmacist went so far as to note not to give me the oval pill.
Now, they can not fill the prescription at all due to the recall. Forgive me but I call Bull*!#T!
I’d really like to know what on earth I was taking!
Any input at all would be greatly appreciated.
    Sincerely,
    John B.

MY QUESTION. Why did Walgreens provide me a refill of this recalled drug on 19 December 2008, were they (Walgreens) not remiss in not pulling the drugs in Sep. 08 Thanks Don

I have been taking metoprolol succinate er for a couple of years for a leaky heart valve. When I called Walgreen’s on Jan. 26, 2009, I was told that the drug was backlogged, and that it might be discontinued. This was the first I had been told of this. I checked your website and found that it had been recalled. I am contacting my cardiologist today for another drug. Thanks for your website!

Don’t be surprised if your doctor is not up-to-speed as regards this Sandoz recall.
Lastly, in case you haven’t heard, ETHEX has recalled their generic metoprolol tablets, now.
To be clear, (at least as I understand the situation) the problem is not about the drug metoprolol but about the fact that these two companies have made their tablets “out-of-specification”, meaning that they have mis-manufactured their metoprolol tablets — and that is why they are being recalled.
Good luck to you, and I hope you continue to read Drug Injury Watch.

Well, well, well with everyone else a resounding Thanks to you for this very needed website. I found out yesterday evening with no meds left that a medication i’ve been taking for 12 years has been recalled, since December and now i have to wait until Feb. to get my refill. I have no meds left and the prescription says do not abruptly stop taking this medicine…so will i have a stroke and not make it to Monday?
I’ve called Walgreen, and rite aid..but after reading these entries i just hung up with Walgreen after being on hold for 30 mins. i’m not sure if i want to restart the medicine… But after 12 years do i have a choice? has anyone found any place where you can get a non-generic replacement? I’ve been experiencing chest pains on the medicine for the past 4-6 months to be honest, i thought it was heartburn, and curtailed my eating habits, started taking anti-acids but perhaps it was the negligence of the mfr.’s of the medicine that produced those side-effects, perhaps that’s why this recall was so hush hush, it’s disturbing that we only have this site to discuss this on, why don’t the mfrs. feel any accountability to us the consumer and thus no responsibility to address us with concrete information about the product we purchase that provides their “bread and butter”. I am appalled quite frankly and too scared of possible side effects to get as mad as i should be at this travesty!

I understand your frustration and concern about this current medication shortage which is due to Sandoz, first, and ETHEX, more recently, having to recall their Metoprolol Succinate ER Tablets because of manufacturing problems at each drug company such that some of these generic pills MAY BE (but currently unknown) out-of-specification, e.g., too little or too much active ingredient.
As a general rule, always be sure to contact your doctor’s office if you feel that you are having any serious side effects when you are taking any prescription medication.

In my opinion, you are justified to complain about the way this generic Metoprolol recall is being handled by different pharmacy stores across the U.S.
What I can tell you — and one would think that your pharmacist knows this and could have told you — is this current shortage of Metoprolol Succinate ER Tablets by Sandoz, first, and ETHEX, more recently, is due to manufacturing problems the FDA discovered at each drug company. Based on what the FDA found during plant inspections, there is a POSSIBILITY that some of these generic pills MAY BE out-of-specification, e.g., too little or too much active ingredient.
To you and everyone, always be sure to contact your doctor’s office if you feel that you are having any serious side effects when you are taking any prescription medication.

I was taking Toprol XL 25mg for 4 years (rapid heartbeat). My mail in prescription service switched me to metoprolol succinate in Sept. I have been having medical problems since October when I started taking the generic. Headaches and sharp pains in my head most severe in the morning. I have seen a neurologist and had extensive testing done with no diagnosis other than possibly migraines. I also thought I might have sinus problems and saw an ENT — No sinus inflammation. I saw an Opthamalgist — eyes fine!
Today when I tried to renew my prescription I was told it was backordered. I tried my local pharmacy who told me about the recall although they would not give me any details. Tomorrow I am picking up a new prescription for Toprol XL (not metoprolol) and hope that my headaches and other ailments cease.

Has anyone had experience with Metoprolol Tartrate made by Mylan? I was transferred from Metoprolol Succinate to the Tartrate after Walgreens informed me Succinate was “no longer available”, no reason given. The doctor didn’t offer a reason either other than Sandoz had a problem with quality – nothing about a recall! What’s with these people?

I too was told by Walgreen’s on Jan. 31, 2009 that the drug had been “recalled” and they had contacted my doctor regarding substitution. I also contacted my doctor’s office. They had been told by Walgreen’s that the medicine was on “backorder”. After finding your website, I contacted my doctor to tell them the product had been “pulled” due to mfg. problems. Sometimes even the doctors are not told.

I’m so angry right now I think my head is about to explode. I have had the exact same experience as several other posters – took Toprol XL for years for irregular heartbeat – heathcare provider changed me to Metoprolol Succinate – went to refill a prescription late last week and when I picked it up, nothing was said to me. When I got home and opened the bag (my fault for not checking before I left Walgreens) noticed the pills are now BLUE and instead of taking 150 mg once a day now I’m taking 100 mg TWICE a day. When I called the pharmacy, I was told that the change was because there is a “shortage”. My doctor had been contacted and changed my medication to Metroprolol Tartrate. I was really irritated that no one had even mentioned the change in medication and dosage. Now – I just came online to look up the correct spelling and happened to find this article. ARE YOU KIDDING ME??? This is a drug for people with heart problems and there is a problem with it that is being handled on the sly??? OMG!! This is unacceptable!! It’s unacceptable that the Walgreens didn’t even mention it to me, it’s unaceptable that my doctor didn’t mention the change to me and it’s REALLY unacceptable to just “happen” to find an article about the drug I have been taking is now being recalled because of quality control issues! No wonder our health care system is such a mess!

I have read several articles about the poor controls of generic medications especially re blood pressure, thyroid and diabetic meds. I spoke to my Dr and required her to write for the brand name. I did notice more headaches on the generic but don’t have that problem now. It’s worth the extra money to know I’m getting the correct dosage. Great website! CVS never told me why the recall was done.

I just called in a re-fill for metoprolol on Saturday and was immediately called by CVS and told that the pills were on back-order; check back on Monday, they said. I went to pick them up today and was told they were “recalled” and that a call had been placed to my doctor for a substitute. I asked for an explanation, however, got none! I asked if I should continue to take the pill; they said that it wasn’t anything major that would warrant a forced stoppage of using the pills. Quite frankly, I’ve had some pretty awful side effects that have been unexplainable until now! I was Toprol XL (and somehow got switched to the metoprolol) to help with my migraines (along w/Topamax), but my headaches have been worse. In November, I ended up in the emergency room getting a cat skan and also having to get 3 shots in my butt (Demerol, nausea meds, and a steroid) because the migraine was so unbearable (missed a week from work). Like several others on this site, I’ve had the crawling skin (on occasion), sore teeth/gums, chest pains, and even problems with my monthly cycle.
This is totally unacceptable and I feel that we as consumers should have been notified in a much better; you get recall letters for your automobiles – why not your medicine? I’ve suffered for 6 months for no reason! I am extremely outraged!!

    Hi there,
I obtain my med, Toprol Xl through a mail order pharmacy. After receiving them for a few months, the next order came in as Metoprolol Succinate. The tablets looked crumbly and after taking a few days worth, I noted the effect was not the same. I called Bio-Script and asked about the change. They informed me that my MD had made it after they contacted him, and that there was no charge for the substituted drug. I insisted that they send me Toprol XL instead, which they did not.THree weeks ago, I placed another order, but did not receive anything. I called Bio-Script two more times, looking for my script to be sent. Not one representative informed me of the recall, but I informed them that one cannot suddenly stop taking a beta blocker due to the risk of irregular heart rate. Today, on February 5th, I phoned the company again and was told about the re-call. My question is, how can a pharmaceutical company keep their customers in the dark regarding the re-call? Users of their product should have been informed. Another question, how can a pharmacy filling the script, just ignore their customers needs?
    I am more than annoyed.

    Dear Tom,
What an eye opener this has been. I have been getting metoprolol ER for a couple years from ExpressScrips out of Arizona and have had a couple different forms. My last batch was from Ethex on Nov. 8 (I guess before any recall by them) but of course noone has contacted me to stop taking them. But luckily I left most of them in Canada by mistake and am now on Par Pharm which is manufactured by Astrazeneca (I called to make sure who they were). On Dec. 29 I had a very severe chest pain that felt like heartburn but I hadn’t eaten much and it lasted for four hours. I never get heart burn. Of course I’m wondering if this pill caused it. I also have had little flutters that last quite awhile but they have gone away since I changed to Par Pharm.
I wondering how to read the label and if any of the numbers might refer to batch numbers. The Ethex recall. had a series of numbers and one of them (369) matched my bottle. Would that be good to know>
Tom, will we find out WHY the recall as it seems like the Sandoz documentation from the FDA was a lot more helpful in knowing what was involved. This one seems so vague.
This seems like something that ought to merit some investigative journalism–i.e. the poor notification and also oversight by the FDA of the process of recall compliance. My doctors have kn own absolutely nothing about this, including a heart specialist.
ExpressScrips sent me an e-mail before the recall saying my prescription request was on hold and they would notify me soon of when I would get more. They have never notified me since and I only found out by demanding to be put through to one of their pharmacists. I would love to know if they ever sold the Sandoz model.
Thanks for your response and that of everyone else.–Dan

    Dear Angie,
I know there are branded alternatives to metoprolol out there, because my father was switched by his cardiologist from metoprolol XL to a new beta blocker. It is supposed to have much less in the way of side effects and appartently better for the heart, as it opens/dilates blood vessels, I guess metoprolol does not . . .at least this is what the doctor explained to us. The price isn’t bad either and he is able to cut them in half too, to save some money. Hope this helps!

I went to CVS today, Feb. 9,2009, and was told that the Metoporlol ER was being backordered and would not be available until the end of the month. The pharmacy told me there was a problem at the warehouse. I CAN’T BELIEVE WHAT I HAVE BEEN TOLD AND THE EXPERIENCES OF OTHER Metporlol ER. CVS never said anything about a recall. It looks like it’s time for legal action……

( 12/04) I began taking Toprol XL at that time, which was changed to Metoprolol Succ. 25mg.
For about 4 months I have been having numerous headaches (usually on the skin surface across my forehead and around the eyes, but sometimes the on he top or back of the head as well.) They are often caused by bending over. It feels like the scalp nerves are sore to the touch.
Also, my gums have been much more sensitive and feel “different.”
Lastly, over the past several years my arms and hands have been tingling almost constantly, with substantial loss of sensation in hands and feet, and a feeling of being cold with chills in the evening, especially during fall-winter-spring. This neuropathy is more severe than my endocrinologist feels is expected for my generally good level of blood sugar control, and he has referred me to a neurologist.
Upon seeing your comments Re: The Recall, I see several similar symptoms by others and wished to add my experiences, for what it’s worth.

HELP!! I was just told by my pharmacist yesterday, 2/9/2009 when I went to get a refill for my metoprolol it was recalled. I was told there is only one generic drug made for Toprol XL.
The manufacturer is listed as Ethex on the prescription label.
    Who is Sandoz?

perhaps the reason that you haven’t been given the Toprol XL brand name is because your insurance will not pay for it…or many pharmacies substitute generic if the prescribing doctor does not specify.

Went to get them filled and was told that they no longer could fill the 100XL’s the were recalled. They did finally fill with the name brand..after I had to make several calls to the doctors to get it…the hospital had no knowledge of the recall(thats what I was told..as I was given generic lopresser while I was there)This is our lives that the drug companies are playing with..I can’t afford to have another stroke..and with the aneurism in the atrial septal portion of my heart I can not have my pulse or B/P elevated.I enjoy your articles..thank-you for being there to guide us…Pam

My husband has been thinking that he was losing his mind. He has been taking Metotoprol for over a year after quadruple bypass surgery. He has been having dry mouth, heart palpitaions, mettalic taste in his mouth, headaches and even pain in his left arm. He has complained of JUST not feeling well. He would get lightheaded and have to lie down at times. He was actually taken to the hospital one night by ambulance and had two other trips to the hospital for the same reasons. We have put out of pocket several hundreds of dollars due to this issue. Someone needs to be held responsible for not only the suffering of physical problems he has endured but also the financial problems this has caused due to having so many visites to the hospital. Has anyone started a class action suite? If not I think it is time that some of us did.

I am soooo outraged. It is February 14th and I am just learning of this only because I went to pick up my Metoprolol ER 100mg (generic for Toprol XL) refill and all the Pharmacist could tell me was that it was pulled. They(Kroger) have since then transferred my prescription to RiteAid but if it is still the medicine I do not want it. I am self pay and unemployed therefore I will have to get it by the days. I knew something was wrong I kept telling my Mom and Dr. I am gaining weight and do not eat much, my hair is falling out and I am tired for no reason. I am unemployed and just tired. I am a fairly active person but can sleep all the time. I just started this Generic about a year ago and I told my Mom and Dr. this is not a good feel. My heart still pounds fast and somedays I am just too tired. My memory is horrible and in the middle of conversation lose my train of thought. Now I am just 34 years old so I know in my heart something is wrong, I just do not have insurance or money to go to a doctor. I too have stumbled across the recall only last Friday.

I have been complaining about this generic form of Toprol XL since the first pill the insurance company forced me to take. I have been back and forth to my cardiologist and internist for months.
Why weren’t we told of this recall? What do we do now? I am contacting the news media to see if they can investigate it.
I am not one to sue, but if a class action suit starts I will be the first to jump on the bandwagon! This has caused me so much harm. Now my future health is in peril! Good Luck to all who have been duped into thinking it was safe. I am ticked off!

Is there any research of toprol to say we should not even take it? This is so depressing – we worry enough about our health without all of this. Please keep up the good work.

I went to the emergency room on Thanksgiving night with warm sensations and then terribly cold sweats. I was at the time taking amlodipine besylate, which seems to be on the wholesale recall list. Anyway, the doctors told me I was fine and that it was just a panic attack. Shortly after I was switched to metoprolol ER and symptoms of fatigue, severe anxiety, and overall just not myself set in. I am now taking xanax for my “anxiety” and it all seems to be linked to this medication. I can’t believe I was prescribed this so late and now I don’t think I should be taking any of this stuff. I told my doctor and the PA my symptoms and thought it was the meds and after five visits they still looked at me like I was a hypochondriac. Is there anyone near Arlington, TX that knows a doc that will listen and find me a BP med that has little side effects, so I can go back to my life.

I called another (Kroger) to see if they had it in stock and was then told the Metoprolol ER (all forms) had been recalled but she could not tell me why but suggested that I contact my doctors office for a generic that could be prescribed. I have been on toprol XL for a couple of years. I started to feel worse when I had to switch from toprol to the generic because my insurance co would not pay for the brand name. I associated my symptoms to stress. I too have chest pains, back pains, shortness of breadth dizziness and I get exhausted just walking across the room. I was angry when I went to refill my prescription and noticed that the pills where different. I called the pahrmacy and they said someone should have told me that they were different generics and the other company was going out of business and recalled the pills. The drug was from Sandoz. Now they gave me

This has me quite ticked off. I’m a physically active 65 yr old in pretty good health taking a 25 mg dose of metoprolol to help control blood pressure. When I went to pick up my prescription this month at CVS (automatic monthly refill) they said they could not get it (pharmacist said he understood that the FDA was unhappy with procedures at the factory and had ordered all shipments halted) and they were faxing doctors to get prescriptions changed.
Today I found your Web site and discovered that whatever was going on at the factory had been a known problem to the FDA at least as long ago as August and that there had been some kind of recall going on since November. I had seen my cardiologist in December and he apparently knew nothing about it because he told me to continue with the same medication and I refilled my prescription in December and January and apparently CVS knew nothing until it was time for my February refill.

My husband has taken Metoptolol Succinate ER 100 MG for about 5 years. His blood pressure was still high 140’s+/90’s. When the drug was recalled his doctor prescribed 100mg twice a day and he became so weak it was hard to walk across the floor. We realized the doctor gave him 100 mg twice and he cut it back to half a pill twice a day and now he only takes half a pill once a day. His pressure is running 120’s/70’s the best it has been in years. I feel like the Metoprolol Succinate ER was like taking nothing at all. I’m glad they recalled it.

I have been taking Metoprolol for several years after my insurance company said they would no longer pay for Toprol XL. The first I’ve heard of a recall was two days ago when I called in my prescription at Walgreens. My pills were also changed from a small one to a large oval shaped pill. I ask the pharacist why and he told me it was the insurance companies idea. They choose which pill I am allowed. I was never told about a recall by my insurance company, doctor, or pharmacy. Why does the FDA not put an article in newspapers. We get a recall notice when our car is recalled, coffee pots are recalled or even toasters. This is my life they are so carelessly considering. There was a huge difference in my general health when my pills were switched, but I just thought my body would get used to it. The FDA is supposed to exist to protect us from this exact neglect. Where are they and where are the answers. When I stop at the Walgreens tonight I don’t know if I have pills or not. I am not supposed to stop suddenly but you tell me where I’m going to get the drug if they don’t have it. I’ll see you in the emergency room or maybe from my grave after a heart attack or stroke. I have notified my children of the issue in case I can’t get pills tonight as I am out.

I am one of many patients that uses the Toprol XL. When trying to have my prescription refilled last month at a Walgreens pharmacy, I was told that they were out of the generic brand ( this is what they gave me over the brand name). Being a faithful user of my medications, I asked why would they call and tell me my prescription was ready and it was not. They refill it with the brand name and sent me along my way. The young lady stated that there was a backlogged on the generic. Today, I called to refill my prescription for this month only to be told that it was recalled. Tom, what does it take to inform consumers of problems, death! I have had hair loss (thinning)and fingers and toes cold. Again, with all the technology out here today, why do the pharmaceutical companies do us this way?

I want to share my exp with my symptoms since going off of Metoprolol ER last Friday. I switched to the name brand, Toprol XL on Saturday morning. I take 25 mg two times a day, one in the morning and one in the evening. I started taking Metoprolol last March, 25MG a day for HBP.
I had been feeling “funny” since taking this drug, but my symptoms got really bad over the last month. I was getting extremely fatigued and struggled with drowsiness. My left arm started getting numbness and tingling in it. I would often get waves of a throbbing, pounding in my head whenever I got up. Then recently I started getting attacks of a very rapid heartbeat, and most recently, I got shortness of breath that was noticeable at all times. I was given Metoprolol for HBP. Before, I never had a problem with my heart, and was very healthy. When I found out about the recall I immediately stopped taking Metoprolol and started on the name brand version Toprol XL.
Since switching to Toprol the head throbbing stopped within the first dose, it is completely gone. I have not experienced numbness in my arm since either. (I can’t say with certainty it is completely gone yet). I have no shortness of breath, that stopped yesterday, and although I am still fatigued, my energy level has come up and everyday it gets better. These changes are directly due to the switch from taking the poison Metroprolol to Toprol.
I did a little research on Sandoz and have found their manufacturing practices to be deplorable. For example, I discovered, TWICE Sandoz was ordered to provide The FDA with samples of their drugs to test and TWICE Sandoz fraudulently passed another manufacture’s drug to the FDA to test and passed it off as their own. The FDA caught them at this. The info is easily accessible on the Internet. Ethex Pharmaceuticals practices aren’t any better. Ethex was caught producing Metoprolol pills that contained up to TWO TIMES the dosage amounts. Both of these companies have a long list of what I believe to be crimes in their manufacturing practices.
I will continue my research and I will make my findings known here.

I have been taking Metoprolol er for a few years and have never had a problem. Taking it for slight Hypertension and Irregular Heartbeats. When going to Walgreens they filled my prescription with name brand Toprol XL for a $25 copay higher than the generic. Was told they were on back order formthe manufacturer. I just got it filled again and told the same thing, nothing about a recall. Course my first thought in these Economic times was that the Drug company just wasn’t making generic inorder to push name brand and increasing the companies profits! Course I still don’t rule this out cause others I know are getting other meds as name brand and told Generic are no longer being produced. If we are forced to use name brand due to recalls and non production, then the insurance companies need to step up to the plate and get these copays lowered for using Name brand. Again I think the others are right that the FDA and Pharmacies should get out there to tell the truth to thier clients ASAP!

    Tom,
I have been taking Metoprolol for about a year now. Since, I have experienced headaches, fatigue to the point of exhaustion, weird numb feelings and much of what the others are saying. Also I noticed the male responses didn’t discuss ED or the lack of interest or sex drive. This was not me a year or more ago. I have never had an issue with this. I’m a Behavior Analyst and have been in my field for over 20 years and train side effects related to the use of phychotropics only. II should have known better than to not question the doc but I stopped the meds about a month ago and am back to normal with staying awake and and not being so tired. My headaches went away also. I was just wondering about the ED issue and if it could possibly be related. Is anyone else having the same issue? It has put a strain on my marriage. My Eurologist prescribed Cialis which is embarrasing but why did he not tell me about the recalls, side effects after completing a med review on me? Any suggestions would be appreciated.

I had been taking metoprolol since last August. I noticed in Nov. that I was having heart rhythm problems. I went to the doctor and she thought I should lower the dose from 100 to 75 mg. a day. I did that and I would have spikes when my heart would race and my blood pressure would rise. Other time my heart would slow way down. I now have A fib. going on and have gone off metoprolol. I sure seemed to have trouble with the metoprolol at this time. I
was to refill my prescription the first of Jan. and the mail in pharmacy called and said they could not get any metoprolol and to contact my doctor for advise. I found it locally and filled it on 1/14/09. I’m not sure if this recall had anything to do with my A fib. but would like to know if anyone had the same problems.

When I asked why they had been recalled, she informed me there was a problem with the absorption rate of the pills. She could not explain further.

I just found out this morning when I called for a refill from my CVS Pharmacy. I asked for specifics from the pharmacists who had none. She said that she had no idea but that they had been cleared off their shelves and I would have to purchase Toprol instead. I have been taking this drug for over a year and a half and the same CVS has provided my prescription refills. Why did they not know prior to today? I am not pleased. I have had my blood pressure checked regularly for additional symptoms I have experienced and my pressure has gone up and down unexplicably. Could this be part of the problem along with other side-effects. What do we do now to find out more. I did not sign up to be a juman guinea pig.

As of yet, the standard web sites, nor the media have mentioned anything about the recall. Interesting! I recently had a heart attack, was given (4) drugs, the statin I eliminated immediately due to side listed effects. I believe we must be responsible for our own health. Well, I am honked off, to say the least. I’m sitting here taking this crap — along with thousands of others — and find out when I try to get a refill on my prescription — along with thousands of others, I suspect — that it’s been “quietly” recalled.
I’ve got a suggestion for the drug companies: Be a little “noisier”in your recalls and a little “quieter” with your ads.
Consumers are bombarded with commercials that extoll the virtues of various convenience drugs instead of being informed in a timely manner that a drug they take that’s CRUCIAL for their health might be a danger to them.
We MUST start holding drug companies responsible for their actions.

The pharm. gave me Lopressor and he told my husband that it was the same med and that instead of 1 a day I needed to take two a day. I have had a bad reaction to blood pressure med. As someone posted earlier and used the expression “I am honked off about this”, it perfectly describes my feelings at learning JUST TODAY of this Metoprolol ER recall! I found out the same way everyone else seems to have, by trying to have my prescription refilled. My insurance co. doesn’t pay for brand names so you can imagine my surprise when I went to pick up my refill and found out it cost $75.00 instead of the usual $15.00. The pharmacy gave me Toprol XL and told me about the “back ordered generic” and the “recall”. When I Googled Metoprolol to find out what was going on, I found this web site. I am soooooo glad I did!!! All the symptoms I’ve read here I have been experiencing quite badly for the last 2-3 months. I have been taking Metoprolol for 1-1/2 years. It was prescribed for me after numerous heart tests to determine the cause of constant palpitations. I never had high blood pressure at all it was low! The pills seemed to work (most of the time) but I would still have times of severe rapid heart beats. In the last few months I’ve also had extreme fatigue, weakness, shortness of breath, the whole nine yards! I thought I was developing some dread disease! I’ve yet to take the Toprol but will start tonight. I’m looking forward to having all these symptoms disappear! I was furious when I had to pay $75 for these pills, but now I’m wondering if it were not a blessing in disguise! Thank you all for your input and for this informative web site.

They need to face civil and criminal charges for not acting in a responsible and professional manner. When a doctor prescribes a drug and finds out is has been recalled, he/she has an obligation to alert his/her patients of such and provide an alternate. The pharmacies also have names, addresses and phone numbers of their customers taking drugs sold be them. They should also be responsible for informing them of a recall.
First and foremost the pharmaceutical companies need to advertise the recall to inform the public.
Shame on them and shame on everyone who believes it is okay to not be accountable for your actions. Passing the buck has contributed to just about every bad thing that is happening in the world today from global warming to the real estate bust and the financial markets collapse. Grow up, take responsibility for your actions!

I am highly ticked off. This entire situation is nothing more to me than the terrorism acts we are suppose to be protected from. I had a stint put in almost two years ago. I had to go on Plavix, Toprol, and Cholesterol medicine. I resisted to the point that my doctor said that with my family history, if I wanted to live, I had to be on meds. Well, through out the past year and a half or so, I have made my doctor change my Cholesterol meds 3 times and am now on an even higher dosage than ever. My reasons for demanding a change is number 1 I am not a pill popper nor do I want to take medicines, and secondly were the symptoms as described in your previous emails. Just the night before last I told my wife that my arms and legs were going numb again and I was scared that maybe my blockage was taking over the stint. I told her last week that I am experiencing hytpertension or faster heartbeats than normal at certain times, she has noticed the exhaustion stages and commented but we thought it was each of the cholesterol meds. Now reading all these reponses I realize that more than likely it was not any of the cholesterol meds but this Metoprolol. They started me on 50 MG of this when I left the hospital and after my three month checkup, I told my doctors that I wanted them to develop me a plan to get me totally off these meds because I am not a pill taker. Well, I can give some if I have to but, when they continue prescribing these type of meds, knowing the circumstances, yes I loose trust. I just got a card out of the blue that scheduled another appointment in early April so I am wondering if this situation is what it is for. Man, can we trust anyone any more? The more research we do, the more we find the government involved to some degrees in unethical practices. Yes, we should hold the FDA accountable but, anyone getting kick backs from the drug companies should also be accountable to some degree. If our doctors really care, then I agree with one of your previous writters, they all have our address information, and with computer systems they can randomly access who is on specific meds with little or no effort. My mother just had heart trouble coupled with other conditions and upon release, she is also another Metoprolol patient. She is 72 years old, so what is this drug doing to her? She has also complained of some of the same symptoms.

I have been taking Toprol XL for 6 years and only recently have I been experiencing some of the symptoms posted here, rapid heart beat, numbness in arms and fingers, tiredness, headaches, lack of ability to concentrate and one that hasn’t been mentioned -pain in my feet. Never thought these were associated with the Toprol. Dr recently switched me to generic of Tenormin because he had heard from his patients that they were unable to get Toprol. When I picked up my prescription from Target I mentioned the recall or low supply to them and they hadn’t heard a word about recall, just a shortage! I agree that the drug companies should be required to publically announce a recall…they did it for peanut butter!

Oh my gosh, I just tried to get a refill on rx only to be told that this medicine was recalled. Why was I not informed. The only reason I am on this is because my health insurance made me go on it as a generic for Toprol. I have lost most of my hair, and I have the numbness in my fingers and my feet.

  86. Joseph Avatar

Hi All, Joseph in San Francisco here.
Thanks for all your feedback! I just learned of the recall today! I use PrimeMail On-line Pharmacy through BlueShield. About 3 weeks ago, I received a letter from PrimeMail stating my Toprol XL order was on “back order” from the supplier; NO MENTION OF A RECALL. The letter said to contact my Dr. for possible alternative.
I’m down to 5 pills. So, I called the Walgreens Pharmacy near my Dr. office to see if they had it before having my Dr. call in the refill. That’s when I learned of the recall. The Pharmacist was very good to provide a lot of detail about the recall. I can’t blame Walgreens as I don’t normally use them, but I’m really irked at PrimeMail.
I’m 45 & had a stint installed @ 40. I’ve taken Toprol XL for 5 years (100 mgs). I had a hard time sleeping, but also take some other meds, so who knows which one is causing the problem. However, my Cardiologist felt comfortable dropping me to 50mgs/day. Reading some side affects here, sounds like Toprol XL may be the cause; headaches, tingles, etc.
I’m an extreme advocate for health care, especially my own, & believe you have to be your best advocate; you’re not happy, speak up and stomp your feet. Your doc ain’t listening, fire them, I did. My last doc got a report from my Neurologist as I had migraines going crazy, learned my BP was through the roof. The Neuro wrote out a great report & plan of action. I literally watched my doc read the report, then hmmd & haad, then file it. I kid you not, I stood up and said “your fired” and left.
I’ve been fortunate to find a great general MD (a woman doc. why do women see the big picture so much better than men?) & a Cardiologist, SF is a God-Send for health care. Both work together for my care.
To those who hate taking medications, I’ve also been given some good not-so-toxic weapons for my heart care arsenal. Not sure if this will help any of you, but my heart doc has me on Omega 3 fish oil capsules (3/day). Helps to lower bad cholesterol naturally. Also I take 1000mg/Niacin a day. You can take over the counter or prescription (known as Niaspan, it’s time released). It’s also a great fighter at lowering bad cholesterol (LDL) & raising the good (HDL). I learned I have friends who have taken Niacin for years, who are not heart patients for it’s known benefits. One issue for men though, for the 1st 4 weeks, you have to endure, hot flashes, skin that feels on-fire, etc. These effects only happen periodically, not 24/7. So, I take mine at night, but take a 81mg/aspirin 30 mins prior.
I made it through my initial adaptation period; LADIES, I NOW UNDERSTAND MENOPAUSE AND HAVE TOTAL RESPECT FOR WHAT YOU GO THROUGH! I had no idea (but I’m a single guy, that’s my defense).
I found my cholesterol levels & BP reacted positively so much faster with these supplements then with medications. I also drink grape juice & pomegranate juice. While being antioxidants they do remove free radicals from the liver, including helping to expel trace medications lingering in your system. Also, cut the salt & keep caffeine reasonable. Caffeine does raise your BP. So, if any of this sounds like it might help you, talk with your Dr.
Now, back to my angst over the recall. Again, being the Advocate, don’t be quick to blame all pharmacies, HMOs, etc, as some may not know about the recall as I found out today. As shared here, some companies have no ethics, so they are sneaky.
I just called my carrier (BlueSheild) and told them of the recall. They were shocked, had NO idea. I could here the rep typing the information as fast as I could provide what I found on-line. She did a search in the company database that yield no info on the recall. She even got my number and asked if the Management Team could call me if they had more questions. She took the report and was forwarding it up the chain of command. As I noted earlier, even my Cardiologist had no idea.
So, I do hold the maker responsible & the FDA!! One oversees the other and that’s where it starts!! I come close to holding PrimeMail responsible, but who knows what they were told; they could have been told it was on back order.
I just e-mail our local news channel guy who does these types of stories. I hope it runs with it.
Again, thanks for all your feedback and hang in there, eat well, get your walks in, and stand up for your health care.
    Good thoughts to you all.

I much appreciate (and others do, as well, I presume) this detailed account of your experience with the Metoprolol recall.
I agree that the parties at fault for the lack of information getting to doctors and pharmacists, much less their patients, are the drug maker and the FDA.

Totally disgusted with all that I have read. I just came from the doctors for treatment of ED. I am a 42 year old male and have been taking Toprol XL for 10 years but the generic version in question for over 3 years at various doses. At one time I was up to 150mg that did not last long because of severe fatigue. Currently I am taking 100mg of Toprol XL 50mg in the morning and 50mg in the evening. I like many others comments was switched to the generic brand because of insurance reasons. Well one day when I went to fill my script I too was told there was a back order for the generic version. The thing that disgusts me the most is the huge circle of fingers pointing the other way. I have been back on the brand name Toprol for over 3 months now and the ED is back along with leg tingles and twitches great now I go to the doctors today and he gives me a script for the “blue pill”. I will be going back to the doctors with these comments from this page and demand something other then Toprol XL. In closing I was put on Toprol to control heart rate when I first went on Toprol I experienced ED at 32. Then as time went on it got better then I was switched to the generic brand and I always felt something was not right this just confirms my thoughts. Now I am back to the name brand and you guessed it the ED is back. Well I got to go because it is time the “blue pill” just kicked in and I would not want to waste my medications. Thank you

I have been taking Metoprolol Succinate ER 50mg for about 18 months and just found out it’s not being made any more when I went for my annual checkup recently and to get the script refilled. My doctor explained that they had found problems with the manufacture, and that they couldn’t guarantee the medicine released in the body at an even rate throughout the day. I will now be taking Metoprolol Tartrate 25 mg twice a day. I, however, experienced NO problems with erratic blood pressure rates (I monitor it at home), headache, or anything else. While I am also disturbed about the issue with the medicine, I doubt if everyone’s problems stemmed from this medicine alone. I do agree that companies–no matter what they manufacture–need to make quality standards a top priority!! The government should use some stimulus money to create inspection jobs in all fields it monitors; food and drugs being on top of the list!

Great! As an update to my last post, I’m now on Metropolol Tartrate 25mg, 2xs/day. In the week and half I’ve been on it, my blood pressure has gone from average 124/68 to 156/100 last night & 148/92 this morning. I’ve been having tightness in the chest, leg pains & arm tingles. I noticed a huge difference within the first few days of taking this med, including massive fatigue.
My doctor’s partner called me when she put this script in, and said “if it causes problems, we’ll switch you to…” I can’t remember which med it was, but I did focus on her saying “if it causes problems.” I’m only guessing, but I presume, most in the med establishment know what the problems might be.

I went from the 200 to 2×100. your symptons sound the same as mine. do you know or have you found out what causes the differences. i dont feel as good as i did only taking once a day, along with the chest tightness and the increase to my previously controled BP.

  94. Joseph Avatar

Dan, I’m still researching on what the difference is that causes these side affects. Fortunately, my best friend recently completed his PhD & has a double masters in Nursing. I mention a drug, and he knows the history. He’s doing some research. I will also be seeing my Cardiologist soon & plan to discuss this with him; he’s a leading cardiologist in CA here in San Francisco. Sometimes, it can be something as simple as the base element they build the drug on.
Sort of odd comparison, but I suffer allergies in spring and use to take prescription Zyrtec, did well on it. Once it became over the counter, it did nothing for me. Yet, they claim is was the same as prescription strength. Well, as for the Mgs strength that may be true, but I discovered the counter version was actually designed differently with different base elements. Something as simple as that can make all the difference in the world.
When I learn more, I’ll post an update.

  95. Joseph Avatar
    Joseph

Dan, already an update. I just reviewed my e-mails and my Cardio’s partner informed me, that with my 25mg M. Tartrate tablets, they actually are absorbed much faster. The old 50mg Toprol XL was better at releasing over time. So, instead of nice 24 hour release, I’m getting (2) jolts of absorption at about 2 hours each. This quick absorption could be the cause of feeling worse.
This may be the case for you, whereas your now taking a 100mg tablets (2x/daily) that is absorbed in a few hours. It is a strong medication and as a result the side affects may be the result.
You should talk with your Dr. about alternative medications. They are out there. One my Dr. is suggesting is Atenolol (trade name Tenormin). It’s been around for years, and I know some who have good success with it. Yet, again, it depends on what other meds you may be taking and how it would interact.
    Hope this helps.

I just opened my mail and then did a search and found your website. The company that manages my prescription drug benefit sent me a leter telling me that Ethex — the company that makes metoprolol ER tablets (I’ve been taking them for about 5 years) “is recalling this medication from all pharmacies and wholesalers. . . . According to the FDA and Ethex, patients who may have metoprolol in their possession should continue to take it” due to the risks of suddenly stopping. I guess my nesxt step is to call my doctor.

Like most, I had no idea Metoprolol ER Succinate was recalled; I was taking 200 daily. My December prescription was replaced with Metoprolol Tartrate 100 x 2 daily; Walgreens said The ER Succinate was on back order. I just refilled my prescription yesterday (5/7/09) was again given the Tartrate; Walgreens said the manufacturer stopped making the Succinate. I was going to write the manufacturer to ask them about it. When I “googled” the medication, I (thankfully) came across this website. Now I know what it going on. I have some of the side effects that others have mentioned, but attributed them to menopause/ depression. I am now concerned that the Metoprolol Tartrate I am currently taking is my problem. I will be seeing my doctor in August and I will suggest twitching to Toprol XL. I am also going to give her a copy of the information provided here.

I was on this drug for two weeks in November of 2008. I felt awful , especially about two hours after taking it. On November 30, I had what might be a heart attack, was diagnosed as Takasuba syndrome.(acute stress cardiomyopathy) My left ventricle balloned after a horrible pounding heart rate. Doctors had no explanation for why this happened. After reading about the recall, I wonder if the drug could be the reason. I am now on Inderal and feel better but still not myself. Of course, i’m on the generic version of inderal. I still worry about the same thing happening again as there is a 10% recurrance rate for TS. If it was the drug, I could rest a little easier.

    Hi All.
It’s been a while since I’ve checked in. I recently contacted the FDA regarding the recall, requested clarification & conveyed issues I have with filling prescriptions. Here’s the reply they sent, may help you all get a better understanding of the issue. It appears Toprol XL brand is available.
    Joseph
Thank you for your inquiry to the Division of Drug Information in Center for Drug Evaluation and Research at the Food and Drug Administration.
There was not a recall on Toprol XL. In fact, it was the generic Metoprolol Succinate that was on recall, which resulted in a short supply of Toprol XL.
Metoprolol Succinate ER has been recalled due to insufficient stability data which is is effecting the expiration date. As a result, many patients transitioned to brand Toprol XL. The alternative is to seek the other generic available by Par Pharmaceuticals.
Please note, there are several issues affecting the availability of Toprol at this time. Toprol XL (metoprolol succinate extended release) is manufactured by AstraZeneca pharmaceuticals. There are two companies who manufacture a generic version of Toprol XL (metoprolol succinate extended release), Sandoz and Ethex. Par Pharmaceuticals, through an authorized generic agreement with AstraZeneca, distributes a generic metoprolol succinate extended release product as well. An authorized generic drug is one that appears to be a generic of a brand name drug, but is actually manufactured and marketed by (or under licensing agreement with) the brand name manufacturer.
The Sandoz and Ethex versions have been recalled in the last several months due to issues with current Good Manufacturing Practices (cGMPs) at their production facilities. Since 2 of the 3 generic versions have been recalled, there has been an increasing demand for the remaining versions (Toprol XL from AstraZeneca and the authorized generic from Par Pharmaceuticals). This increased demand has led to an overall shortage of product in the market. Both Par and AstraZeneca state that they have product available and are allocating it as it is being produced.
It is important to contact various pharmacies in your area to see if they have available products since many large chain and supermarket pharmacies use different wholesalers and distributors.
If, after reviewing this information, you have any unanswered questions, please contact the Division of Drug Information by phone (1-888-463-6332) or e-mail (druginfo@cder.fda.gov). Thank you again for your message.

Hi Glen, I frequently post here, going to see my Cardiologist today to get off the M. Tartrate. I can’t tolerate the side affects anymore. Call’d various pharmacies & Toprol XL & the one official generic recognized by AstraZenaca (makers of Toprol XL) are still in short supply due to the other two unofficial generics being recalled (see my post below from the FDA).
Meanwhile, I hear you on the pill-popping. I have no knowledge of your diet, but in my prior posts, you’ll see I advocate diet as a big helper with Cardio health. Yet, I firmly believe if you feel you can make dietary changes for the better, do so, while on the meds. Once you have a diet plan in place, then discuss with your Dr. about transitioning off meds & see the test results as your diet continues. I know we all hear it daily about the benefits foods such as: berries, fish, walnuts, spinach, asparagus & low or no salt. Here in San Francisco, many restaurants are cutting salt from dishes, it’s very noticeable & I appreciate their efforts, plus there are so many spices to cook with other than salt.
I also tell folks all the time, in today’s environment, you really do have to be your best health care advocate. If you don’t like what you’re hearing, question it by all means. The one good thing about enforcing yourself on a diet is it’s not odd for a combination of meds to cause issues as well, and there are times fine tuning is required.
I’m like you, hate taking pills, So a year ago, I really focused on my diet & what I was eating, and more importantly, what I should be eating. I’ll be honest, it’s been hard; my weakness in life is CHEESE, ANY KIND OF CHEESE.
Hang in there and keep us posted.

Since starting on Toprol I have slowly developed periferal neurapathy. The nerves in both feet are numb and feel like pin pricks. (like novicain) Also swelling of feet. Since toprol effects the nervous system, I suspected a link. My doctor denies any connection and prescribed another nerve drug meant for diabetics, which I dont have. What is the latest on metoprolol succinate ER tabs?
Walgreen’s Pharmacy just refilled a prescription with the generic version manufactured by PAR Pharmaceuticals. I took one pill and had side effects which were not evident in the previous 6 months while I was on brand name Toprol XL. In answer to my inquiry, the Walgreen pharmacist said there never was a recall just a back order problem! She also said emphatically that the side effects I had (palpitations and irregular pulse) were not caused by the medication even though they occurred 3 hours after taking the first pill of the new batch. I have requested my doctor change my prescription back to the Toprol XL which the pharmacist said would cost me over $90 because my insurance probably will not cover the brand name medication.
