Zithromax-Induced Liver Injury Case Report Where There Was No History Of Liver Problems

Drug-Induced Liver Injury (DILI), Or Toxicity, Can Cause Both Acute And Chronic Liver Disease

To get our bearings, we start with some points taken from the Abstract for the article "Diagnosis, management and prevention of drug-induced liver injury", which was published in the Gut medical journal last year:

  • Drug-induced liver injury (DILI) is increasingly being recognised as a significant cause of both acute and chronic liver disease.
  • Drug-induced hepatotoxicity is generally idiosyncratic in nature.
  • Diagnosis of drug-induced liver diseases can be difficult, but the key to causality is to diligently exclude other causes of liver injury, and to identify a characteristic clinical drug-related signature.

Next, we consider the recent case report about a 69-year-old woman developed cholestatic hepatitis after four days of therapy with high-dose Zithromax (azithromycin) for the treatment of suspected bronchitis.

From the Abstract for the article "Azithromycin-induced liver injury", published earlier this year in the American Journal of Health-System Pharmacy we get these key facts:

  • After receiving four days of high-dose [Zithromax (azithromycin)] for the treatment of suspected bronchitis, a 69-year-old woman arrived at the emergency room with nausea, vomiting, diarrhea, elevated liver enzyme values, and visible signs of pruritus and jaundice.

  • Causes of primary liver injury, including metabolic, viral, and autoimmune liver diseases, were excluded.

  • After exclusion of other causes of liver disease, drug-induced hepatotoxicity was considered. A careful review of her medications prior to admission was conducted. A temporal relationship between initiation of [Zithromax (azithromycin)] and the onset of clinical signs and symptoms was noted. The Naranjo et al. probability scale indicated a possible relationship between azithromycin and hepatotoxicity; however, two scales specifically used for evaluating drug-induced liver disease indicated a probable adverse drug-associated event.

Zithromax, sometimes called "Z-Pak", is a popular antibiotic manufactured by Pfizer that is used to treat a variety of bacterial infections.  In recent years Zithromax has been associated with not only hepatitis (as in the case report, above) but also more serious types of liver damage such as liver failure and necrosis of the liver, medical conditions which may require a liver transplant or result in death.

The possibility of liver damage from Zithromax arises because Zithromax, like the other antibiotics in the macrolide class of drugs, is primarily eliminated from the body through the liver.

P.S.  As of February 2011 the FDA is continuing to evaluate liver failure as a potential drug safety issue for Zithromax to determine the need for any regulatory action.

    After taking zithromax , my friend arised some problem in their throat? Is it right, zithromax causes throat problem?

    At the present time I am unaware of any Zithromax side effect involving one’s throat, but that is not to say that your friend’s medical condition is unrelated to their use of Zithromax.
    I suggest your friend speak with the doctor who prescribed the Zithromax in order to find out if their throat problem may be a Zithromax side effect, or not.

    How long should I wait before I drink alcohol after taking 5 days of zithromax? I’m headed on vacation soon and don’t want to risk anything.

    I strongly suggest that you ask your doctor this question — or, at least, read the Prescribing Information or other documentation that came with your Zithromax.

    Hi Tom, I have taken zithromax z-pak for a number of years with no problems then I took azithromycin – generic from Teva in August and all hell broke loose. I too had swallowing problems, heart palipations, aniexty through the roof – I have never been anxious before, distonia in my neck and head, gastritis, mucscle cramps, shortness of breath, sensitivities to food, insomina – did not sleep for 3 weeks straight feeling that I was dying. I had many other symptoms to numerous to mention here. I have been a strong healthy person all my life. Then in August through Jan 2011 I spent over 12,000 trying to figure out what I had with over 30 drs. and many tests – got another cold in March and dr gave me generic azithro. again I took it for 2 days only and my smyptoms came back again with a vengence – so at least I know what did it but now I have broncitis and pnuemonia which I never had before – now on predinsone – hate it – It is like this drug made me allergic which again never been before. Many others I have spoken too have been on the brand name one with no problems but had many of the symptoms I have had on the generic. The generic has a different molecular structure than Pfizer’s brand and different inactive ingredients – also made outside US – less gualify control there – I and the people I have talked too feel as if we were poisoned – maybe a contaminant. Or the different structure or ingredients cause this – we the public think a generic is exactly the same as the brand and as a result should have same experience but the drug is different. Have you heard of these problems with the generic and can there be a possiblility of it being contaminated? I would love to have the pill anaylized. Do you know any attornyies that I and others would be able to talk to about this. Thanks for your help Valerie

    Thank you for sharing your experience with generic Zithromax (azithromycin).
    I have not heard from anyone else about the possible contamination of these azithromycin pills made by Teva.
    As far as generics being made differently — as opposed to exactly the same — as the brand name drug, there has been alot of discussion about this type of issue as regards Teva’s generic Wellbutrin product. See:

    I’ve taken z-pack numerous times over the past 2+ years for sinus trouble and on a recent sonogram of my abdomen saw a mass in my liver. Could it be possible that this was caused by z-pack? Would hope to hear from you about this.

    As you can see from the article above, the side effect possibly associated with Zithromax was liver injury and liver failure.
    I am not a physician so I cannot answer your medical question about what the mass is much less whether it may be a related side effect. I suggest you make this inquiry to one of your doctors.

    My cousin is a 37 year old male. Good health, never really been to the hospital, never really gets sick…….two weeks ago he saw a doctor complaining of flu like symptoms…..the doc perscribed a z-pack and two days later he found himself in the ICU with Acute Liver Failure…..a week later the biopsy showed 68% of his liver is dead and he is currently on the transplant list…the doctors are hopeful he will recover without needing a liver transplant, but we are still in a holding pattern with the transplant list. There is no family history of liver disease and doctors have ruled out hepatitis, auto-immune deficiencies and liver disease. This has been a life changing event for my cousin and my entire family…..what legal actions can we take against pfizer?

    I am a retired ICU Registered Nurse. Three years ago, I was hospitalized for pneumonia and received 5 days of IV Zithromax, followed by 5 more days of oral Zithromax after discharge. Within a week after completion of the medication, I was exhibiting very dark rusty urine. My doctor checked my Liver Function tests and they were 20 times normal. I had never had any elevation in the past.
    After almost six months and a liver biopsy etc., the LFTs were still at least 10 times normal. The liver biopsy showed bridging fibrosis. I began research of the medical literature, and found that Milk Thistle is frequently listed as a possible treatment for liver disease. Within a month of starting it, my LFTs were only a few points above normal. My goal is to prevent further liver damage, and includes avoidance of all alcohol (I rarely drank in the past), and avoidance of Tylenol and any other medication with possible liver side effects. I suggest others research the benefits of Milk Thistle. I know several MDs who recommend it for their patients on potentially liver-damaging medications.

    I took zithromax for 4 days and became very ill. I noticed the whites of my eyes turned yellow and my biiliubin test was off the charts.
    My liver enzymes were extremely high. I could barely eat and now
    I am left to worry about liver function . I have to go for a cat scan to see how bad the liver is damaged . All from taking an antibiotic Zpak for infection!

    Hopefully you will be getting better now that your are off the Zithromax.
    As you can see from the other Comments, your incident was not isolated. However, this side effect is serious and should be discussed more by prescribing doctors with their patients.

    My husband is 4 th stage kidney failure doctor wants to give him Zithromax for pneumonia is this a good thing seeing as he is on rocephin for a uti

    I am a 26 year old male, always been healthy never had a problem with my health in my life. Last year at the age of 25 I came down with bronchitis and went to cvs walk in where the doctor prescribed be 5 days zpak instead of ten to rid me of the virus, little did I know antibiotics can’t kill viruses because viruses are not alive. After taking the zpak I woke up the next day with massive amounts of floaters in my eyes, weak like my blood had stopped flowing properly and tingling in my fingers. If anyone knows anything about the blood in our bodies then you know it all gets filtered by the liver. Zpak ruined my liver completely, I could not sleep for almost 2 months straight, I became extremely paranoid and depressed, contemplated suicide multiple times because I could not dea with my situation. After going to more than 30 doctors and them not finding anything I took my life into my own hands and started Chinese natural herbal remedies and massages on my self, I became my own guinea pig. After almost 2 years of constant overload of probiotics and natural foods I can say that I am back to 80% of who I was before I took this devil of a medicine called zpak. The liver becomes stagnant after taking zpak, and the main thing that helped me was probiotics and apple cider vinegar, apples have a substance in them that almost repairs the liver and it greatly helped me along with daily massages of the liver. Also getting back in the gym and getting your blood flowing helps greatly. I know it’s tough to get up after such a devistating thing happening to you but you have to, if you don’t do it no one will. I went from 3-4 hours of gym/basketball a day before zpak to not even being able to life my head and losing 20 pounds of muscle, now I’m back at the gym just pushing myself constantly regenerating my own liver cells. The human body is something miraculous that I can’t explain but I do know that what you put into your body can change it for better or worse and in my case I was able to reboot my liver and start from the bottom. I’m still climbing uphill and I won’t stop until I reach the person who I was before taking zpak. I wish everyone a speedy recovery and just hang in there and be aggressive in your rehab, even if you feel like stopping just keep going because you will see the light at the end of the tunnel just as I did.

    My best friend had a swollen throat and flu like symptoms so I took her to the doctors office. They saw her for about 4 min and prescribed her the generic zpack. She went home and took the first two pills. I came by later and she said she was feeling a little better so I went home. I got a call yesterday from her roommates, they found her dead in her bed the next morning. I’m completely lost without her and I am waiting for the autopsy report to see if this anti biotic has killed her. She has 4 kids and was the greatest woman ever… Absolutely the worst thing that’s ever happened to me in my life. Please let me know if anybody else has died from this drug, thanks.

    My late husband died from an “adverse reaction” from taking a zpack. At least that is what the autopsy revealed. He was a healthy 52yo. He came home with only a sore throat and nausea and vomiting. 2 days later he saw a doctor and was prescribed a zpack. After 3 days he went into acute hepatic failure, which led to being placed on a vent, kidneys shut down, 24 hour dialysis, increased cranial pressure led to a bolt being placed in his skull. He was tested for everything under the sun. All results came back negative. I ended up withdrawing life support after 4 days….Dec 29,2012. As you can see by the date of his passing, it was on christmas day….

    Hello, Two weeks ago last Monday I starter with a cough and after a few days it had gotten worse so I completely stopped smoking. By the next Monday I had built up chest congestion so I thought I’d better go see my Doc.. Well, she put me on Azithromycin and a steroid. By the next day (Tuesday) I was very sick to my stomach. Come Wednesday I called the pharmacy and told them my symptoms and they said it was all normal. By Thursday I was wrenching over the toilet and so very very sick to my stomach.
    By Saturday by daughter took me to the ER. They really couldn’t figure out what the problem was but game me Zofran for nausea and a breathing treatment for my congestion and sent me home. By Monday I had experienced hives allover my body both Sunday and Monday morning so I dragged my sick body to the doc and found I had lost ten pounds from the week before and my lab tests showed a complete upside from the week before.
    My doc was acting scared and said she wanted me to see a pulmonary doc ASAP. The next day I drove my sick body an hour away and did the breathing tests, xrays and gave more blood. Today she wanted me to drive two hours away to see the pulmonary doc to get the results and have him treat me. I was so sick this morning I called and cancelled the pulmonary appt. and asked them to please send results to my doc.
    My doc called me and screamed at me that I should of made that pulmonary appt. and that she’s the doctor not me. I told her how sick I am and she said I need to take the anti-nausea medication and made the appt.. I stressed to her that the day before I took the medicine and I was still very sick the entire trip and by the time I got there I was completely covered in hives again.
    My doc had told me last Monday that she was going on vacation starting this Friday so I just feel she want’s to hand me off to a pulmonary doc so she feels better leaving me sick and in another docs hands.
    I know my liver is the problem. I have had the dark urine, major nausea, my skin looks so so dry, and the heaving over the toilet. This morning in the beginning I felt normal but as the morning went on I could feel the tightness or inside swelling in my gut, the nausea coming on and the wrenching over the toilet.
    I asked my doc what my lab work showed regarding my liver enzymes and she said they never looked at my liver. My white blood count had jumped up, my ESR (SWOLLEN) results went from 35 to 95 and my kidney only slightly increased from 1.1 to 1.2. That’s all she could tell me. She did say I was very sick and still insists I see the pulmonary doc.. I insisted she get the results from the pulmonary doc and lets talk but I cannot go that far being this sick.
    I am hoping by the end of this week this stuff will be pushed out of my body but I am at a loss to what my next move needs to be. Any ideas? I am terrified time will continue to move like this and it will be to late.

    hi tom i have a question they gave me 1 zithromax pill with a steroid pill at the hospital i had pneumonia and i got a prescription for 4 more pills for the next 4 days to kill the lung infection in my lungs i was coughing very violently – i called walgreens and asked them if a small does is ok she told me yes – is it ok for a small amount of the pills to be taken to prevent liver problems?
