New Heart Failure Drug Entresto Apparently Has Safety Signal For Hypotension (Low Blood Pressure) Side Effect

Will Recent ISMP QuarterWatch Report Prompt FDA Regulatory Action, Including Possible Drug Label Change By Novartis?

The FDA approved Entresto™ (sacubitril and valsartan) tablets in July 2015 to reduce the risk of cardiovascular death and hospitalization for heart failure in patients with chronic heart failure (NYHA Class II-IV) and reduced ejection fraction.

In the original and still current Prescribing Information for Entresto (Revised 8/2015 and accessed 11/21/17), at Part 5 “Warnings and Precautions”, already has some text and data about incidents of low blood pressure in some patients using this rather unique heart failure drug:

5.3 Hypotension

ENTRESTO lowers blood pressure and may cause symptomatic hypotension. Patients with an activated renin-angiotensin system, such as volume- and/or salt-depleted patients (e.g., those being treated with high doses of diuretics), are at greater risk. In the double-blind period of PARADIGM-HF, 18% of patients treated with ENTRESTO and 12% of patients treated with enalapril reported hypotension as an adverse event [see Adverse Reactions (6.1)], with hypotension reported as a serious adverse event in approximately 1.5% of patients in both treatment arms. Correct volume or salt depletion prior to administration of ENTRESTO or start at a lower dose. If hypotension occurs, consider dose adjustment of diuretics, concomitant antihypertensive drugs, and treatment of other causes of hypotension (e.g., hypovolemia). If hypotension persists despite such measures, reduce the dosage or temporarily discontinue ENTRESTO. Permanent discontinuation of therapy is usually not required.

Despite this cautionary language, the November 1, 2017 edition of ISMP QuarterWatch, a publication from the Institute for Safe Medication Practices, reports that a so-called “drug safety signal” has been detected recently for Entresto as regards hypotension being a serious side effect which warrants more attention — and, perhaps, and an increased warning.

Relying upon “New data from 2017 Q1” of the U.S. Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS), this leading drug safety publication ends its recent report on Entresto with these statements in the final Conclusions section:

This large volume of reports of hypotension confirms the concerns of FDA reviewers that the [Entresto (valsartan-sacubitril) clinical trial results in carefully selected patients underestimated the risk of hypotension in normal clinical practice. As we noted, the risk is so prominent that about 1 out of 4 patients started on [Entresto] will experience a hypotension-related event.

We recommend that the FDA and [Novartis Pharmaceuticals Corporation] the manufacturer [of Entresto] more clearly communicate the full risk of hypotension, including appropriate steps for monitoring patients, and examine whether a more gradual escalation of dose to the recommended level would reduce the risk.

This November 2017 ISMP QuarterWatch Entresto safety analysis article provides more detail about the number of hypotension reports made to the FDA in the section titled “The FAERS Data”:

In the 12 months ending March 2017, we identified 6,770 adverse drug event reports for [Entresto (sacubitril-valsartan)], including 681 (10%) patient deaths on one extreme, but 4,131 (61%) cases that were coded as occurring but with no medically serious consequences. The reported adverse reactions included medically important adverse events also seen in the large clinical trial, including renal failure and impairment (n = 317 PRR = 3.19), angioedema (n = 152 PRR = 5.63), and hyperkalemia (n = 87; PRR = 11.51). However, the most frequently reported adverse reactions were hypotension-related events (n = 1,684 PRR = 4.8).

At an easier to understand level, this new Entresto drug safety issue is presented in the Executive Summary part of this November 2017 ISMP QuarterWatch publication as follows:

[T]he signal investigated for this report suggested that many patients starting on this new drug [Entresto] were experiencing hypotension (low blood pressure), and the reported complications ranged from dizziness to blackouts and other consequences serious enough to require hospitalization. We identified 1,684 adverse event reports indicating a hypotension-related event, more than for any other cardiovascular drug we monitored over the 12 months ending in 2017 Q1. They occurred in older patients (median age 70 years), and although there were 69 reported deaths, in two-thirds of the cases, the health consequences were not severe.

Further investigation showed this adverse event was known, but likely underestimated in the single large clinical trial that supported [Entresto’s] FDA approval. In that study, 24.4% of carefully selected patients experienced a hypotension-related adverse event. But FDA reviewers noted this was likely an underestimate because this enriched trial was conducted only with patients already known to tolerate the study drugs without experiencing adverse events severe enough to cause discontinuation.

We will be watching the see whether the FDA takes any regulatory actions concerning Entresto or whether Novartis requests permission from the FDA to make a label change strengthening the warning about the apparent increased risk of this hypotension Entresto side effect.

    Entresto was added to my daily medications and now within 3wks of taking it I have severe gout in my left foot and I can’t walk. I also developed a soft cough and flu like diarrhea. I have stopped the Entresto 3 days ago and gout intensity is diminishing. Cough and diarrhea is still somewhat there.

    My husband passed away in December. He was on Entresto. The doctor started him on the low dose. He moved to the mid range dose. Went back to Dr. because he feltl bad. PA increased dose and sent him for blood work. Kidney function numbers were high and they dropped dose back to low dose. He continued to deteriorate. His EF readings fluctuated between 29% on August 31,2018 to 35% on September 28th. Was made an appointment to see Dr. About implantation of ICD on October 4th 2018. He wanted to do a MUGA test. Scheduled that for October 17th . He continued to deteriorate. Saw Dr on the 5th of November5th for follow up to MUGA test. Were told that EF was 36% and was not a candidate for ICD. Over all this time Dr was increasing and decreasing his diuretic. Deteriorated more and went back to see cardio Dr. on November 20 2018. He scheduled another echo for November 27th. Never made it to that. Went to ER on November 22. Kidneys were failing and heart was failing. Very quickly his kidney functions jumped to Creatinine level of 6.1. Never recovered and he passed away on December 11 2018. This has been devastating. Not sure what I can do to bring the dangers of this medication to light

    I had twins 6-11-18 and was a stage 4 hemmorage i lost 90% of my blood volume. And now i guess i have pregnancy cardiomyopathy. I was put on metoprolol
    And lisiniprill however when my ef dropped from 57%to 35% they took me off lisiniprill and put me on entresto. I’ve only been on entresto since March 2019. I seen my Dr last Tuesday and mind you today is may 22,2019 but last Tuesday my Dr took a blood sample because my muscles are on fire everything I did took all my strength and well with 4 children i can’t afford to be down. He called me on Friday and said my muscle enzimes were elevated and my kidneys were starting to fail. Also hibigloben was low. So he told me to stop taking entresto as soon as possible. My cardiologist didn’t believe it could be the medicine as the risks was only a 0.02% or so chance of happening so they blammed it on my emotional well being so my primary is the one who treated my emotional well being and had me come back 3 weeks later to test and found out about my muscles and kidneys and since I stopped I’ve been walking on my gazelle and going for walks I still hurt my muscles aren’t back to normal but I was also freezing on the entresto but now I can regulate body temp my cough went away, I feel 100x better then I have in a while. I shouldn’t even be here to tell my story but for the first time in a long time or so it feels like I can get out of bed and want to do daily cares and be the best I can be for my children and go on walks with my husband and children. I was scared I wasn’t going to make it to the twins birthday which is less then a month away and now I feel like I’m feeling better mind you after my heart Cath I had fluid around my heart and only pump 4.5 litters instead of 5 but that’s during entresto I was put on spinolactone and still taking my metoprolol so I’m still protected but feel so much better. Like I at least know I bought myself some time because I went against my cardiologist and went to my primary Dr for help and to see if we can find answers. And he believed me and found out through a thorough blood test that I was in fact telling the truth. Cardiologist just do a mild blood testing as for a in dept thorough test for everything it may help you out before its too late!

    I am heartfailure patient. 8 years since mi. Have UCD and pacemaker. Functional class 2. EF 35 %. Was well adjusted on standard HF protocol. Tried adding Entresto and soon suffered severe bouts of hypotension post excercise. ER once after cardiac rehab. Called it quits after two months. Feel much better now back on old 2009 recommended drug protocol. I guess I was just not the right candidate for Entresto.

    I was on Entresto, and had to go to er 3 times for low pressure. The third time I was driving home on I20, blood pressure dropped, I went in coherent and totaled my truck. I thank God that I didn’t hurt or kill someone or myself. The ER doctor stated that my blood pressure dropped so low that my brain lacked oxygen. He also stated it was the entresto. I quit taking it, and now me blood pressure is normal, and i feel much better and have more energy.

I thank God that I didn’t hurt or kill someone or myself. The ER doctor stated that my blood pressure dropped so low that my brain lacked oxygen. He also stated it was the entresto. I quit taking it, and now me blood pressure is normal, and i feel much better and have more energy.

    Them increase to 2 a day. He is not feeling well his legs are very swollen. This medicine is helping his heart according to all his Doctors. But is affecting his kidneys
    Every time I talk to the Doctor about this medication and the water pills. They just said well that’s the only thinks helping your husband to be alive!,,
    Is very frustrating situation. I ask the to decrease to just 1 pill a day but his heart failure dr said NO”
    Any way. Don’t know what to do

    He is still facing low pressure knowing that he is having 60 mg of lasix.
    Should i wait more for his blood pressure to be adjusted and back to normal?

    I had heart failure and ef was 17%. I wasn’t expected to survive the night. I used Reiki and determination. Last reading was 65% but I feel cold, no energy at times, muscles weak and my Acupuncturist tells me my kidney pulses and very weak and my blood volume is low. I’m a holistic practitioner and work on myself daily. You have given me the motivation to stop taking Entresto.

    I had heart failure and ef was 17%. I wasn’t expected to survive the night. I used Reiki and determination. Last reading was 65% but I feel cold, no energy at times, muscles weak and my Acupuncturist tells me my kidney pulses and very weak and my blood volume is low. I’m a holistic practitioner and work on myself daily. You have given me the motivation to stop taking Entresto.

    I thank God for all the comments. My dr started me on entresto 2 years ago and I was so exhausted and hurting in my legs that I took an early retirement from my job. I was on losartan and was doing good now my heart function has went from 45 to fluctuating between 30 and 35. I told my cardiologist that I stay so exhausted all the time and she said that it was the congestive heart failure but I didn’t feel like this until I started the entresto. Time for a second opinion!

    I thank God for all the comments. My dr started me on entresto 2 years ago and I was so exhausted and hurting in my legs that I took an early retirement from my job. I was on losartan and was doing good now my heart function has went from 45 to fluctuating between 30 and 35. I told my cardiologist that I stay so exhausted all the time and she said that it was the congestive heart failure but I didn’t feel like this until I started the entresto.     Yes I have been on entresto for about 1year and I loss the ability to get out of bed and my lower back hurts can’t go to bathroom to pee have to pee but can’t couldn’t get no answers on why this was happening so took myself off the drug can finally pee again still some discomfort don’t know what kind of damage has been done so would like to know what I can do about this

    Incredibly refreshing to read complaints and symptoms for what I thought were (and have been told by cardiologist) psychological. I have been on Entresto for two years. Orthostatic intolerance symptoms came on immediately. Before this medication I was on my legs twelve plus hours a day and extremely active in spite of cardiomyopathy. I am now on disability and completely inactive.
    Thanks to everyone’s comments I am through with Entresto.

Orthostatic intolerance symptoms came on immediately. Before this medication I was on my legs twelve plus hours a day and extremely active in spite of cardiomyopathy.     Entresto has sent me to ER twice after both initial low doses. I have yet to get it on board. Be very cautious with this med. In the clinical trials done by Novartis the maker of Entresto, the patients were chosen by Novartis that used people that they already knew could tolerate their drug. The clinical trials weren’t even completed. Even after this drug has caused deaths, mostly from hypotension, there is still no warning from the FDA or Novartis. It may also do serious damage to your kidneys. This is no miracle drug as it is touted by many. For some it is a big improvement, for others the side effects can be a nightmare even dangerous. Do your homework with this drug,talk to your Pharmacist. The right one can be a wealth of knowledge. Be sure and start with a low dose. I don’t care what your Cardiologist says. Don’t start off with two a day. Try one pill and see what happens. Monitor your BP every hour. It is notorious for severe hypotension. Be certain your hydrated and the correct amount of salt in your bloodstream. Wear compression stockings. If things go bad(my BP went to 85/37 on the low dose before 911) lie down and elevate your feet above your heart and start sipping water and call 911. Good luck.

    Has anybody had Kidney Issues? Went to Cardio Visit Bloodwork showed Creatine level was 9. Hospitalized, immediately taken off Entresto but told not cause of Kidney Failure. No cause determined for Creatine #. Started Dialysis now down to 7.4 after almost 3 months not optimistic about kidneys coming back/restarting. Now looking at permanent port for Dialysis.

He had bundle branch block on the left side but had controlled the symptoms with other medications for YEARS. Four days after using Entresto he passed away in his sleep with NO warning. He passed away on Valentine’s Day. Please use extreme caution with this drug!

    My sister took this and she was on it for about two years. Her doctor told me that she needed this drug to keep her out of hospital. At the time she had no problems . After she started taking it I notice she started to get weak. She got weaker as time went on. He blood pressure got so low. Also she had very low blood count. She went from a healthy person to me taking care of her 25 hours a day. Her Kidneys started shutting down and she was hospitalized 2 ,weeks and here blood count went down to 19. They told me to take her home with hospice and she would not live very long and see passed away 4months later all because of this poison drug.

    Yes, I’ve been on it for 2 months now and I’m having short term memory loss. Like I walk into a room get something and then I don’t remember getting it. I know I did because I have it, but for the life of me, I do not remember going into the room and picking it up. It’s strange and concerning.
