FDA Mandates Label Changes With New Warnings For Invokana, Farxiga, Jardiance, Other SGLT2 Inhibitors

In late August 2018 the FDA warned doctors and patients that cases of Fournier’s gangrene, a rare but serious infection of the genitals and area around the genitals, have been reported with the class of type 2 diabetes medicines called sodium-glucose cotransporter-2 (SGLT2) inhibitors. More specifically, this serious rare infection is called necrotizing fasciitis of the perineum.

Oral diabetes medicines included in this SGLT2 inhibitors drug class are Invokana, Farxiga, and Jardiance, as well as Invokamet, Xigduo, Glyxambi, Synjardy, Qtern, Steglatro, Steglujan, and Segluromet.

From this August 29, 2018 Drug Safety Communication, "FDA warns about rare occurrences of a serious infection of the genital area with SGLT2 inhibitors for diabetes", we get the following details about this new safety issue for Invokana, Farxiga, Jardiance, and the other SGLT2 inhibitor diabetes drugs:

Fournier’s gangrene is an extremely rare but life-threatening bacterial infection of the tissue under the skin that surrounds muscles, nerves, fat, and blood vessels of the perineum….

In the five years from March 2013 to May 2018, we identified 12 cases of Fournier’s gangrene in patients taking an SGLT2 inhibitor…. Although most cases of Fournier’s gangrene have previously been reported in men, our 12 cases included 7 men and 5 women. Fournier’s gangrene developed within several months of the patients starting an SGLT2 inhibitor and the drug was stopped in most cases. All 12 patients were hospitalized and required surgery. Some patients required multiple disfiguring surgeries, some developed complications, and one patient died. In comparison, only six cases of Fournier’s gangrene (all in men) were identified in review of other antidiabetic drug classes over a period of more than 30 years.

In the Additional Information for Patients part of that FDA document there is this important point:

  • Seek medical attention immediately if you experience any symptoms of tenderness, redness, or swelling of the genitals or the area from the genitals back to the rectum, and have a fever above 100.4 F or a general feeling of being unwell. These symptoms can worsen quickly.

We have started investigating possible drug injury lawsuits for patients diagnosed with necrotizing fasciitis of the perineum, also referred to as Fournier’s gangrene, while using Invokana, Jardiance, Farxiga, or one of the other SGLT-2 inhibitor diabetes drugs.

    I have been taking Farxiga for about 3-4 months and now have some kind of infection in the area of my groin and scrotum. The skin gets moist and has an odor quite like feces. I visited with my GP and he prescribed Cipro for 10 days and have been using ketoconozole cream along with Lamasil powder up to two and three times a day. My problem is that my skin at the junction of the scrotum and top of the inside of the thigh is “weeping” or oozing again and I have only been off of the Cipro for 1 day. I can’t find any photos of the problem you are representing patients for so I have nothing to compare it to. Should I get a swab of this and have it analyzed? Any thoughts on this?

I had the entire right half of my groin amputated up into my right hip and down past the muscle 3 years ago because of a rare form of necrotizing fasciitis. I’m STILL recovering. I’ll never be normal again. This situation has changed my life forever.
    I had 3 emergency surgeries and over 27 debridements. I was on life support for three days and in the hospital for over 2 and a half months. Then had to spend 4 months at my parents because my dressings needed to be changed twice a day until I was healed which took 6 months. I’m a type two diabetic with a very rare autoimmune disorder called Behçets disease. I was on 2000 MG of metfor,in each day with 4mg of Amaryl and insulin as needed.     I take Jardiance. I have the same problem. I have what looks like a 2 inch scar where it started now it flares up around it. I have shown it to 4 doctors and they say it looks awful but don’t know what it is. I’m about to find out. It is awful.

    I was taking invokana for 2 years and immediately stopped after I was heaving bile for a month. My dr then put me on Jardiance 2 years ago and for the last 4 months have had a horrible rash down below with crackling of the skin. After telling him all he said was Use the over the counter yeast meds which I already did. I have been in agony with these rashes and went to a gynecologist who in turn said she had no idea what it was and prescribed a steroid cream which at first I thought it was working. Been using it for almost a month and it seems he rash is just coming back. My question is how does this gangrene start? Anyone else go through this rash thing first?

    Hi beth, mine started as a little lump the size of a pea. It didn’t hurt, it was movable and for the most part I wasn’t worried about it until like a monthat after I saw it… one night it just got angry and blew up in size and my right half of my groin was hugely inflamed and there were three distinct black blisters on it. I took photos and went to throw ER. I hope that helps, and if I were you, I’d go to the ER.

    I have been on Met Forman and Jardi ants for years I have had intermittent infections rash on my penis and related area that is very painful, painful to the touch dark purple in color, swelling, incredible pain on urination skin dryness and cracking leaving scars around the foreskin area I have had three episodes of this in the last year or so. My Met Forman has been reduced to 1000 due to a septic infection E. coli in the bloodstream and urine, I survived the five day hospital stay on massive doses of antibiotics and that special cream that takes care of gangrene and other symptoms that were found on my genital area. I don’t know if these are all related and I was not hospitalized for the genital infection but I was given medications by my primary care physician, I don’t know if all of these symptoms are related i.e. the septic E. coli infection of urine and blood and the associated confinement in the hospital are related to the urinary track infection possibly caused by Met Forman and Joe audience. It was painfully debilitating each time I had the symptoms and they last for about 7 to 10 days do I have a case

    My brother died last oct.30th.he had diabetes type 2 for years I believe he was on metformin.one day he calls me crying that he is going to die,he had gone to the doctor and was told he had to go in the hospital ,his legs hurt so bad he could hardly walk,he said he could see muscles and blood but no flesh and he finally had his left leg amputated after that he was told the right one would need to be amputated too.but he only lasted 2-3 days and he passed away ,I believed it was some sort of flesh eating decease,anyone have any idea what it could have been,my brother was all I had left ,I do miss him.

This drug is deadly and shouldn’t be on the market.

    I’m having the same issues
    I found a small movable bump and can’t gey rid of these awful rashes. I’ve used all sorts of over the counter meds. I’ve taken Jardiance since heart surgery on 2017. All of this just started recently.

    Yes, it sounds similar to my experiences. While several doctors didn’t really know, they all said they thought it was probably a yeast infection, but I’ve never had a yeast infection like this & it has never yielded the usual symptoms. The yeast cultures all were Negative. So no answers & a LOT of pain. I’m about to actively pursue some answers, because my tolerance for the pain has run out.
    I’d like to ask if there’s bleeding associated with your symptoms?

    Yes, it sounds similar to my experiences. While several doctors didn’t really know, they all said they thought it was probably a yeast infection, but I’ve never had a yeast infection like this & it has never yielded the usual symptoms. The yeast cultures all were Negative. So no answers & a LOT of pain. I’m about to actively pursue some answers, because my tolerance for the pain has run out.
    I’d like to ask if there’s bleeding associated with your symptoms?

    Hi Beth. I was on Jardiance for about 6 months. Suddenly I developed a sore throat. Had it checked right away; to possibly get ahead of it. Doc gave me 6 Prednisone for over 3 days and a throat rinse. Throat became worse. Then went to my usual doc. She said I had foot/mouth disease. Gave me a Zpak for 5 days. No relief. Then the throat became so bad I couldn’t swallow. I could see the yellow infection in the back of my throat, and it went very deep. I gagged terribly. Couldn’t drink or eat. Wound up in the ER for dehydration due to vomiting/dry heaves. My rash was just beginning in my groin area. This all started 7/23. Now, 8/18, and its difficult walking due to the rash and burning. I have tried many things for relief but none has come. I’m pretty sure it’s bacterial at this point. And I’m scared. I stopped that Jardiance in July, and my blood sugars have been 117-205. Trying to control it with diet. No sugar. No carbs to speak of. My pharmacy person is seeing me tomorrow about this.

    I’m on Januvia does that cause gentian gangrene ? My Dr says it doesn’t but I’ve have severe groin and leg pain bilaterally but just had a right hip replaced 10 weeks ago

    The Pliva, Inc., et al. v. Mensing case was decided by the Supreme Court of the United States (SCOTUS) on June 23, 2011. The bottom line is that the US Supreme Court ruled in favor of the generic drug company and against the injured patient such that at the present time any “failure to warn” lawsuit involving a generic drug would be dismissed based on this Mensing case ruling.
