More Than 1 Million Packages Of Defective Devices Used To Check INR For Warfarin May Cause Serious Injuries Or Death

In September 2018 Roche Diagnostics issued a voluntary recall of more than 1 million packages of the CoaguChek XS PT Test Strips that were sent out nationwide from January 2018 up to the date when Roche issued this CoaguChek recall.

On November 1, 2018, however, the severity and urgency of this CoaguChek INR test meter devices recall was increased when the FDA designated it a Class I recall. For medical devices, this is the most serious type of FDA recall, reserved for situations where use of the defective devices may cause serious injuries or deaths.

As background, CoaguChek XS PT Test Strips are used with Roche INR Test Meters to monitor patient response to warfarin (also known by the brand names Coumadin and Jantoven), providing the test result in the form of the International Normalized Ratio, or INR. The INR is used by patients and physicians to determine if warfarin dosing is appropriate.

In January 2018 Roche re-calibrated the CoaguChek XS PT Test Strips in to correspond to a newly released INR International Standard. According to the FDA, in the months following this re-calibration Roche Diagnostics has received reports of patients experiencing abnormally high or inaccurate INR test results when testing with CoaguChek strips.

The FDA CoaguChek XS PT Test Strips Class I recall announcement includes this serious message: "Use of affected products may increase the risk of serious adverse health consequences, including death." It goes on to provide this additional important information:

Patients with the following conditions are at especially high risk for serious events associated with inaccurate INR measurements:

  • Mechanical heart valve
  • Atrial fibrillation and high-risk CHA2DS2-VASc scores
  • Recent thromboembolic events

    I have been using the Coagu test strips. In August, I had a bleed into my right leg. My readings
    Went from 6 to one within a few days. It made it difficult to get my dosage correct.

    My husband and I on Coumadin and experienced high readings compared to the doctors office. We have no test strips to monitor at this time.

    Good afternoon,
    I wanted to reach out to you regarding my CoaguChek text strips. I have 96 strips that I have paid out of pocket for. All have been recalled. I am on Coumadin for life due to a blood clotting disorder. I have tried to contact Rocha several times. I left 3 voicemails with no response. After my 4th time calling, I was able to talk to a live person. He informed me that they will not replace my straps because I paid for them out of pocket and I did not get them from one of their insurance distributors. I have to test my blood at home twice a week or more if I’m feeling ill. Stress makes my INR go up (thin blood) and I’m at risk for internal bleeding. I’m not sure if you can help me but I really do need some help. This is a life or death situation for me. Thank you

    I am very upset about this recall i have paid out of pocket for these stips and now told they will not replace them . I test twice a month and could not fiqure out since purchasing these stips my inr has been off. I can’t afford to repurchase and why do we need to go thur roche now to there program to beable to test. I have been able to travel and not be affarid of my inr being off because i could check it and call it in. I will worry now! Are they doing this just to control us and collect the money we will pay for there program. Pleas help

    I suggest that everyone who has not received a callback from Roche Diagnostics regarding the replacement of CoaguChek Test Strips that were paid for and now those test strips will not be replaced, consider a Class Action Lawsuit against Roche Diagnostics.
    Roche Diagnostics was informed by Hospitals and Coumadin Clinics that testing discrepancies were being found, and Roche Diagnostics failed to inform the FDA and Point of Care Testers of these proclamations from Hospitals and Testing Laboratories.
    When I became aware of the “Test Strip False Readings” from my Coumadin Clinic, I contacted Roche Diagnostics directly, and without requesting a replacement Testing Unit, was sent a Replacement CoaguChek Test Unit and Test Strips that were then “Listed on the FDA Recall of Roche Diagnostics CoaguChek Test Strips”.

    I am on coumadin for life due to a clotting disorder. I had a knee replacement in September and had postoperative bleeding twice. Due to that bleeding I had to be wrapped tight to avoid a blood transfusion which hindered my bending of the knee. Now it’s December and I am still struggling with my knee. If my levels were correct I probably would have never had this bleed. I hope a class action law suit is filed.

    Are you sure Roche supplied all the lot numbers of the defective strips? I started getting unusual readings on my CoaguChek XS, one week it would be 1.9 and the next week it would be
    3.4. I went to the hospital lab and had an IV blood INR taken. The CoaguChek reading was 3.3 and the
    blood draw at the lab was 2.2. The last test I took was 2.8 with the CoaguChek and 2.2 at the hospital lab with an IV blood draw INR. The Dr. said not to use the CoaguChek anymore. The lot numbers of my strips didn’t match the defective strip numbers sent to me by Roche. In September and October I was having all the variations in readings and
    I ended up having a heart attack in Oct.. My strips are not included in the recall but they were reading higher than the lab readings. I think there were more defective strips than Roche is reporting.

    Andrea, were your test strips on the “affected lot numbers” list? Mine weren’t on the list but test results were always higher on the CoaguChek than a blood draw at the lab. I discovered this after I had a heart attack and the Dr had me check the difference in the CoaguChek and a IV check at the lab? I was having my Warfarin dosages adjusted according to the CoaguChek which was inaccurate. Yet, the lot numbers of my strips were not on the affected lot list. I think there are more strips that were defective than the lot numbers listed by Roche.
